1. Indications and Usage
Glycemic Control in Type 2 Diabetes (Brand: Mounjaro):
Indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
Chronic Weight Management (Brand: Zepbound):
Indicated for chronic weight management in adults with obesity (BMI ≥30 kg/m²) or overweight (BMI ≥27 kg/m²) with at least one weight-related comorbid condition.
Limitations of Use:
Not indicated for use in patients with type 1 diabetes mellitus or for the treatment of diabetic ketoacidosis.
2. Dosage and Administration
Route of Administration:
For Subcutaneous Injection ONLY. Administer once weekly, at any time of day, with or without meals.
Recommended Dosage:
Initial Dose: 2.5 mg once weekly for the first 4 weeks (treatment initiation dose, not intended for glycemic control).
Titration: After 4 weeks, increase the dose to 5 mg once weekly. If additional glycemic control is needed, increase the dose in 2.5 mg increments after at least 4 weeks on the current dose.
Maximum Dosage: 15 mg once weekly.
Missed Dose:
If a dose is missed, administer it as soon as possible within 4 days (96 hours) after the missed dose. If more than 4 days have passed, skip the missed dose and administer the next dose on the regularly scheduled day.
3. Mechanism of Action
Dual Receptor Agonist:
Tirzepatide is a GIP and GLP-1 receptor agonist. It activates both GIP and GLP-1 receptors.
Glucose-Dependent Insulin Secretion:
It enhances glucose-dependent insulin secretion and reduces glucagon levels.
Weight Loss Effects:
It delays gastric emptying and influences satiety and appetite, resulting in decreased food consumption and body weight reduction.
4. Safety and Warnings
Thyroid C-Cell Tumors (Black Box Warning):
Tirzepatide causes thyroid C-cell tumors in rodents. It is contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC) or in patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
Pancreatitis:
Acute pancreatitis has been reported. Discontinue promptly if pancreatitis is suspected.
Gallbladder Problems:
Cholelithiasis and cholecystitis have been reported in clinical trials.
Hypoglycemia:
Risk increases when used with insulin secretagogues (e.g., sulfonylureas) or insulin. Consider reducing the dose of these concomitant medications.
Acute Kidney Injury:
Renal impairment has been reported, often secondary to dehydration from severe gastrointestinal adverse reactions.
Hypersensitivity Reactions:
Anaphylaxis and angioedema have been reported. Discontinue use if a serious hypersensitivity reaction occurs.
5. Adverse Reactions
Most Common:
Nausea, diarrhea, decreased appetite, vomiting, constipation, dyspepsia, and abdominal pain.
Gastrointestinal Events:
Gastrointestinal adverse reactions are the most frequent side effects, occurring primarily during dose escalation and generally decreasing over time.
6. Drug Interactions
Oral Medications:
Due to delayed gastric emptying, tirzepatide may impact the absorption of concomitantly administered oral medications. Caution is advised for drugs with a narrow therapeutic index (e.g., warfarin).
Insulin Secretagogues:
Concomitant use increases the risk of hypoglycemia.
