Duoliwei Clopidogrel Bisulfate and Aspirin Film-coated Tablets

1. Indications and Usage
Secondary Prevention: Indicated for secondary prevention in adult patients who are already taking both clopidogrel and aspirin for atherothrombotic ischemic events (e.g., prior myocardial infarction, stroke, or established peripheral arterial disease).
Acute Coronary Syndromes (ACS): Indicated as a conversion therapy for patients with ACS (non-ST-segment elevation acute coronary syndromes including unstable angina/non-Q-wave myocardial infarction, and ST-segment elevation acute myocardial infarction who are suitable for thrombolysis) receiving concomitant clopidogrel and aspirin.
2. Dosage and Administration
Recommended Dosage: One tablet taken orally once daily, which contains 75 mg of clopidogrel and 100 mg of aspirin.
Administration: The tablet should be swallowed whole and not crushed or chewed. It can be taken with or without food.
Treatment Duration: In non-ST-segment elevation ACS, clinical trials support a treatment duration of 12 months. In ST-segment elevation myocardial infarction, treatment should last at least 4 weeks.
3. Mechanism of Action

CYP2C19 Inhibitors: Omeprazole and esomeprazole significantly reduce the conversion of clopidogrel to its active metabolite, decreasing antiplatelet efficacy. Pantoprazole or rabeprazole are preferred alternatives for gastroprotection.
NSAIDs and Anticoagulants: Non-steroidal anti-inflammatory drugs (NSAIDs) and anticoagulants (e.g., warfarin) significantly increase the risk of gastrointestinal bleeding.
Serotonergic Drugs: Selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) may impair platelet serotonin uptake and increase bleeding risk.
