1. Indications and Usage
Secondary Prevention: Indicated for secondary prevention in adult patients who are already taking both clopidogrel and aspirin for atherothrombotic ischemic events (e.g., prior myocardial infarction, stroke, or established peripheral arterial disease).
Acute Coronary Syndromes (ACS): Indicated as a conversion therapy for patients with ACS (non-ST-segment elevation acute coronary syndromes including unstable angina/non-Q-wave myocardial infarction, and ST-segment elevation acute myocardial infarction who are suitable for thrombolysis) receiving concomitant clopidogrel and aspirin.
2. Dosage and Administration
Recommended Dosage: One tablet taken orally once daily, which contains 75 mg of clopidogrel and 100 mg of aspirin.
Administration: The tablet should be swallowed whole and not crushed or chewed. It can be taken with or without food.
Treatment Duration: In non-ST-segment elevation ACS, clinical trials support a treatment duration of 12 months. In ST-segment elevation myocardial infarction, treatment should last at least 4 weeks.
3. Mechanism of Action
P2Y12 Inhibition: Clopidogrel is a prodrug that requires metabolism by CYP450 enzymes (primarily CYP2C19) to form an active metabolite. This active metabolite irreversibly blocks the P2Y12 ADP receptor on platelet surfaces, preventing fibrinogen binding and inhibiting platelet aggregation.
COX Inhibition: Aspirin irreversibly inhibits cyclooxygenase-1 (COX-1) in platelets, thereby blocking the synthesis of thromboxane A2, a potent inducer of platelet aggregation and vasoconstriction.
4. Safety and Warnings
Severe Hemorrhage: The combination dramatically increases the risk of major bleeding, including intracranial and gastrointestinal hemorrhage (e.g., peptic ulcers). Contraindicated in active pathological bleeding.
Hypersensitivity: Contraindicated in patients with severe hypersensitivity to NSAIDs (e.g., aspirin-exacerbated respiratory disease with asthma, nasal polyps, or anaphylaxis).
Liver and Kidney Impairment: Contraindicated in severe hepatic insufficiency or severe renal impairment.
5. Adverse Reactions and Clinical Research
Most Common Adverse Reactions: Bleeding (gastrointestinal, retinal, gingival), gastrointestinal disturbance (dyspepsia, abdominal pain), diarrhea, and skin rash.
Clinical Research: Clinical data confirm that dual inhibition of ADP receptors and COX-1 synergistically reduces cardiovascular events (myocardial infarction, stroke, vascular death) compared to monotherapy, with peak efficacy observed at 3 months and plateauing at 12 months.
6. Drug Interactions
CYP2C19 Inhibitors: Omeprazole and esomeprazole significantly reduce the conversion of clopidogrel to its active metabolite, decreasing antiplatelet efficacy. Pantoprazole or rabeprazole are preferred alternatives for gastroprotection.
NSAIDs and Anticoagulants: Non-steroidal anti-inflammatory drugs (NSAIDs) and anticoagulants (e.g., warfarin) significantly increase the risk of gastrointestinal bleeding.
Serotonergic Drugs: Selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) may impair platelet serotonin uptake and increase bleeding risk.
7. Pharmaceutical Information
Chemical Composition: Active ingredients are Clopidogrel Bisulfate (75 mg) and Aspirin (100 mg).
Appearance: Typically yellow or off-white round or capsule-shaped film-coated tablets, depending on the manufacturer.
Storage: Store at 25°C (excursions permitted 15-30°C) in a dry place and protect from light.
