1. Indications and Usage
LCH: Indicated for the treatment of adult patients with Langerhans cell histiocytosis (LCH) and histiocytic tumors.
NF1: Indicated for pediatric and adolescent patients 2 years of age and older with symptomatic, unresectable plexiform neurofibromas (PN) associated with neurofibromatosis type 1 (NF1).
2. Dosage and Administration
Recommended Dosage: For adults, the recommended dosage is 8 mg taken orally once daily. For pediatric patients (2-17 years), the recommended dosage is 5 mg/m² taken once daily.
Administration: The tablets should be taken orally once daily, with or without food.
Dose Adjustments: Treatment should be continued until disease progression, unacceptable toxicity, or physician guidance to discontinue.
3. Mechanism of Action
MEK Inhibition: Luvometinib is a selective inhibitor of MEK1 and MEK2 kinases.
Pathway Blocking: It blocks the MAPK/ERK signaling pathway, thereby inhibiting tumor cell proliferation and inducing apoptosis.
4. Safety and Warnings
Cardiac Monitoring: Left ventricular ejection fraction (LVEF) should be regularly evaluated, especially in patients with cardiovascular risk factors.
Ophthalmologic Monitoring: Ophthalmologic examinations should be performed to monitor for retinopathy, such as retinal vein occlusion or retinal pigment epithelial detachment.
5. Adverse Reactions and Clinical Research
Common Adverse Reactions: In pediatric NF1 patients, the most common drug-related adverse reactions (incidence ≥20%) include increased LDH, increased CPK, paronychia, α-hydroxybutyrate dehydrogenase elevation, oral ulcers, rash, and sinus arrhythmia.
Clinical Significance: In clinical trials for LCH and NF1-PN, no significant decrease in LVEF was observed in patients treated at the recommended dosage.
6. Drug Interactions
UGT Metabolism: Luvometinib is primarily metabolized by uridine 5′-diphospho-glucuronosyltransferases (UGT1A3 and UGT2B17).
Enzyme Inhibitors/Inducers: Concomitant use with strong UGT inhibitors or inducers may significantly alter luvometinib exposure.
