1. Indications and Usage
Indicated for the treatment of patients with mild cognitive impairment or mild dementia due to Alzheimer’s disease.
2. Dosage and Administration
Dosage: The recommended starting dose is 300 mg taken orally once daily.
Titration: The dose may be increased to 600 mg once daily after at least 3 months of treatment, based on clinical assessment and tolerability.
Administration: Can be taken with or without food. The tablet should be swallowed whole and not crushed, divided, or chewed.
Missed Dose: If a dose is missed, it should be skipped, and the next dose should be taken at the regular scheduled time. Do not take two doses on the same day.
3. Mechanism of Action
Lemercutib is a selective inhibitor of beta-secretase 1 (BACE1). By inhibiting BACE1, it blocks the proteolytic cleavage of the amyloid precursor protein (APP), thereby reducing the production of amyloid-beta (Aβ) peptides. This mechanism aims to slow the progression of Alzheimer’s disease pathology.
4. Safety and Warnings
Hepatic Toxicity: Elevations in liver transaminases (ALT/AST) have been observed. Liver function tests should be monitored periodically before and during treatment.
Skin Toxicity: Cases of severe skin reactions, including Stevens-Johnson syndrome, have been reported. Patients should be advised to report any rash or mucocutaneous lesions immediately.
CNS Effects: Some patients may experience confusion, agitation, or other central nervous system abnormalities.
Pregnancy: Use is not recommended during pregnancy and lactation due to potential risks to the fetus or infant.
5. Adverse Reactions and Clinical Research
Common Adverse Reactions: The most frequently reported adverse reactions include elevated liver enzymes, skin rash, headache, nausea, and fatigue.
Clinical Research: Phase II/III clinical trials have demonstrated that lemercutfib significantly reduces CSF and plasma levels of Aβ42. However, its efficacy in improving cognitive function scores compared to placebo is still under rigorous evaluation in ongoing studies.
6. Drug Interactions
CYP2D6 Inhibitors: Strong inhibitors of CYP2D6 may increase plasma concentrations of lemercutfib. Dose reduction may be considered if co-administration is necessary.
CYP2D6 Inducers: Strong inducers of CYP2D6 may decrease plasma concentrations of lemercutfib, potentially reducing its therapeutic effect.
Other BACE Inhibitors: Concomitant use with other BACE inhibitors is not recommended due to the lack of safety and efficacy data.
