1. Indications and Usage
Indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
2. Dosage and Administration
Recommended Dosage: The recommended dose is 10 mg taken once daily in the morning, with or without food. In patients who tolerate this dose, the dose may be increased to 25 mg once daily.
Renal Function: Do not initiate if eGFR is below 45 mL/min/1.73 m².
3. Mechanism of Action
Empagliflozin selectively inhibits sodium-glucose cotransporter 2 (SGLT2) in the kidney. This inhibition reduces renal glucose reabsorption and lowers the renal threshold for glucose, thereby increasing urinary glucose excretion and lowering blood glucose levels.
4. Safety and Warnings
Hypersensitivity: Serious hypersensitivity reactions (e.g., angioedema, Stevens-Johnson syndrome) are contraindications.
Volume Depletion: Risk is increased with concurrent diuretic use, elderly patients, or renal impairment. Correct volume depletion before initiation.
Ketoacidosis: Skeletal DKA may occur; consider discontinuation during acute illness, fasting, or reduced caloric intake.
Renal Toxicity: Monitor for acute kidney injury and worsening renal function.
Infections: Increased risk of genital mycotic infections and urinary tract infections.
