1. Indications and Usage
Diabetes Mellitus: As a second-line treatment for adult patients with type 2 diabetes mellitus who have not achieved adequate glycemic control with diet and exercise alone.
2. Dosage and Administration
Route: Oral administration.
Dosage: Standard dosage is twice daily. The maximum daily dosage must not exceed the recommended limits for individual components.
Timing: Take during meals to minimize gastrointestinal discomfort.
3. Mechanism of Action
Metformin: A biguanide that decreases hepatic glucose production, decreases intestinal glucose absorption, and improves insulin sensitivity.
Vildagliptin: A dipeptidyl peptidase-4 (DPP-4) inhibitor that increases the levels of incretin hormones, stimulating glucose-dependent insulin release and inhibiting glucagon secretion.
4. Safety and Warnings
Lactic Acidosis: A rare but serious complication associated with metformin accumulation; risk increases in renal impairment or hypoxic states.
Hypoglycemia: Risk may increase when combined with insulin or sulfonylureas.
Hypersensitivity: Severe allergic reactions, including angioedema, may occur.
Hepatic Impairment: Use with caution in patients with liver dysfunction.
