1. Indications and Usage
Hemorrhagic Conditions: Indicated for the treatment of excessive bleeding due to increased fibrinolytic activity (e.g., in liver disease, hypermenorrhea, epistaxis, and bleeding during surgery).
Hereditary Angioedema: Used for the prevention of attacks in patients with hereditary angioedema.
2. Dosage and Administration
Adults: The usual dosage is 500–1000 mg (2–4 tablets) three to four times daily, taken orally.
Duration: For acute bleeding, it is typically used for a short period until the bleeding is controlled.
Adjustments: Dosage may need to be reduced in patients with severe renal impairment (creatinine clearance <30 mL/min).
3. Mechanism of Action
Antifibrinolytic Effect: Aminomethylbenzoic acid is a synthetic derivative of the amino acid p-amino benzoic acid.
Inhibition of Plasminogen: It competitively inhibits the activation of plasminogen to plasmin, thereby preventing the breakdown of fibrin clots (antifibrinolysis) and stabilizing formed clots.
4. Safety and Warnings
Thromboembolism: Can cause or exacerbate thromboembolic events (e.g., deep vein thrombosis, pulmonary embolism, cerebral or coronary thrombosis). It is contraindicated in patients with a history of these conditions.
Subarachnoid Hemorrhage: Should not be used for treating subarachnoid hemorrhage.
Renal Function: Since the drug is excreted by the kidneys, accumulation may occur in patients with renal failure, increasing the risk of seizures.
5. Adverse Reactions and Clinical Research
Common Adverse Reactions: Nausea, vomiting, diarrhea, and abdominal discomfort.
Serious Adverse Reactions: Thromboembolic events, myocardial infarction, and seizures (particularly in patients with renal impairment or high doses).
Other Effects: Visual disturbances and skin rashes have been reported.
6. Drug Interactions
Combination with Prothrombin Complex Concentrates: Concurrent use with prothrombin complex concentrates, activated prothrombin complex concentrates, or antifibrinolytics may increase the risk of thrombosis.
Oral Contraceptives: May increase the risk of thromboembolic disorders when used with oral contraceptives.
Anticoagulants: Antagonist effect with heparin or warfarin.
