1. Indications and Usage
Etrasimod is indicated for the treatment of adults with moderately to severely active ulcerative colitis.
2. Dosage and Administration
Recommended Dose: 2 mg taken orally once daily.
Administration: Can be taken with or without food.
Titration: Unlike some drugs in its class, etrasimod does not require dose titration.
Missed Dose: If a dose is missed, take it as soon as remembered on the same day. Do not take two doses on the same day.
3. Mechanism of Action
Etrasimod is a selective sphingosine 1-phosphate (S1P) receptor modulator. It binds to S1P1, S1P4, and S1P5 receptors. By doing so, it inhibits the egress of lymphocytes from lymph nodes into the bloodstream, thereby reducing the migration of inflammatory cells to the gut mucosa and lowering intestinal inflammation.
4. Safety and Warnings
Cardiac Monitoring: Can cause initial dose-related bradycardia and atrioventricular conduction delays. ECG monitoring is recommended for the first 6 hours after the first dose.
Infections: Increases risk of infections (e.g., herpes, cryptococcus). Screen for VZV and TB before starting.
Hepatotoxicity: Elevations in liver enzymes have been observed. Monitor liver function tests.
Macular Edema: Can cause cystoid macular edema. Ophthalmologic monitoring is recommended.
Malignancy: Increased risk of skin malignancies; perform skin exams.
5. Adverse Reactions and Clinical Research
Common Adverse Reactions: Headache (9%), elevated liver enzymes (6%), dizziness, nausea, arthralgia, hypertension, and urinary tract infection.
Clinical Research: Efficacy was established in the Phase 3 ELEVATE UC 52 and UC 57 trials, showing significant clinical remission and mucosal healing compared to placebo.
6. Drug Interactions
Cardiac Drugs: Caution with Class Ia or III antiarrhythmics (risk of QT prolongation).
Beta-Blockers: Concomitant use with beta-blockers may have unknown additive effects on heart rate.
CYP Inhibitors/Inducers: Moderate CYP2C9/CYP3A4 inhibitors (e.g., fluconazole) may increase exposure; strong inducers (e.g., rifampin) may decrease exposure.
