Storage: Store under refrigeration at 2–8°C (36–46°F). Do not freeze. Keep in the original carton to protect from light. 1. Indications and Usage
Rheumatoid Arthritis (RA): Treatment of adult patients with moderately to severely active RA, in combination with methotrexate (MTX) or as monotherapy.
Juvenile Idiopathic Arthritis (JIA): Treatment of patients 2 years of age and older with polyarticular JIA.
Psoriatic Arthritis (PsA): Treatment of adult patients with active PsA.
Ankylosing Spondylitis (AS): Treatment of adult patients with active AS.
Plaque Psoriasis: Treatment of adult and pediatric patients (6 years and older) with moderate to severe chronic plaque psoriasis.
2. Dosage and Administration
Route of Administration:
For Subcutaneous Injection ONLY.
Recommended Dosage:
Adults (RA, PsA, AS): 50 mg once weekly OR 25 mg twice weekly (approximately 72 to 96 hours apart).
Adults (Plaque Psoriasis): 50 mg twice weekly for up to 12 weeks, followed by 50 mg once weekly.
Pediatric Patients (JIA): 0.8 mg/kg per dose (up to a maximum of 50 mg per week), administered once weekly or as two divided doses of 0.4 mg/kg twice weekly.
Administration:
Patients may self-inject if their physician determines it is appropriate and they receive proper training.
3. Mechanism of Action
TNF Receptor Antagonist:
Etanercept is a dimeric fusion protein consisting of the extracellular ligand-binding portion of the human 75 kilodalton (p75) TNF receptor linked to the Fc portion of human IgG1.
Blocking Signaling:
It binds with high affinity to soluble TNF-α and lymphotoxin-α (LT-α, also known as TNF-β), preventing them from interacting with cell surface TNF receptors. This neutralizes the biological activity of TNF, thereby reducing inflammation and tissue damage.
4. Safety and Warnings
Serious Infections (Boxed Warning):
Patients are at increased risk for serious infections (e.g., Tuberculosis, bacterial sepsis, invasive fungal infections) that may lead to hospitalization or death. Screen for latent TB prior to initiation.
Malignancy:
Increased risk of malignancies, particularly lymphoma and leukemia. Cases of hepatosplenic T-cell lymphoma (HSTCL) have been reported in adolescents and young adults.
Hepatitis B Reactivation:
Cases of HBV reactivation have occurred in carriers. Monitor liver function and HBV viral load.
Nervous System Events:
New or worsening central or peripheral nervous system demyelinating disorders (e.g., Multiple Sclerosis, optic neuritis) have been reported.
Hematologic Events:
Rare cases of pancytopenia, including aplastic anemia, have been reported.
Live Vaccines:
Avoid concurrent use with live vaccines.
5.Drug Interactions
Anakinra / Abatacept:
Concomitant use is not recommended due to an increased risk of serious infections and no additional clinical benefit.
Tocilizumab:
Concomitant use is not recommended due to increased risk of serious infections.
6.Adverse Reactions
Most Common:
Injection site reactions (rash, pruritus, erythema, pain, swelling) are the most frequent adverse event. Other common reactions include upper respiratory tract infections, headache, and diarrhea.
