1. Indications and Usage
Adrenal Insufficiency (AI): Replacement therapy for primary and secondary adrenocortical insufficiency (e.g., Addison’s disease).
Congenital Adrenal Hyperplasia (CAH): Salt-wasting forms of congenital adrenal hyperplasia.
Orthostatic Hypotension: Occasionally used off-label for the treatment of severe orthostatic hypotension.
2. Dosage and Administration
Administration: Oral administration, usually taken in the morning.
Starting Dose:
Adults: Generally 0.1 mg once daily.
Children: Generally 0.05 to 0.1 mg once daily.
Titration: The dose is adjusted based on clinical response (blood pressure, electrolytes) and plasma renin activity.
Dietary Measures: Patients may require increased dietary sodium intake, especially during stress or diarrhea.
3. Mechanism of Action
Synthetic Mineralocorticoid: Fludrocortisone is a synthetic corticosteroid with potent mineralocorticoid activity and some glucocorticoid activity.

4. Safety and Warnings

5. Adverse Reactions

6. Drug Interactions
Hypokalemia Risk: Concomitant use with amphotericin B, loop diuretics (e.g., furosemide), or thiazides increases the risk of hypokalemia.
Glucocorticoid Activity: Concomitant use with other steroids (e.g., prednisone) increases the risk of glucocorticoid side effects (e.g., peptic ulcers, hyperglycemia).
