1. Indications and Usage
Metastatic Non-Small Cell Lung Cancer (NSCLC): Indicated for the treatment of patients with metastatic NSCLC with progression on or after platinum-based chemotherapy.
Unresectable or Metastatic Melanoma: Indicated for the treatment of patients with unresectable or metastatic melanoma.
Recurrent or Metastatic Head and Neck Squamous Cell Carcinoma (HNSCC): Indicated for the treatment of patients with recurrent or metastatic HNSCC.
2. Dosage and Administration
Recommended Dosage:
Dose: 3 mg/kg intravenously.
Schedule: Every 2 weeks.
Administration: Intravenous infusion over 60 minutes.
Dose Modifications:
Immune-Mediated Adverse Reactions: Interrupt or permanently discontinue based on severity (Grade).
3. Mechanism of Action
PD-1 Receptor Antagonist:
Nivolumab is a human immunoglobulin G4 (IgG4) monoclonal antibody that binds to the PD-1 receptor.
Physiological Effect:
It blocks the interaction with its ligands (PD-L1 and PD-L2), thereby reversing the PD-1 pathway-mediated inhibition of the immune response, including the anti-tumor immune response.
4. Safety and Warnings
Immune-Mediated Pneumonitis: Can occur and may be fatal. Monitor for respiratory symptoms.
Immune-Mediated Colitis: Monitor for diarrhea and abdominal pain.
Immune-Mediated Hepatitis: Monitor liver function tests (ALT, AST, Bilirubin).
Embryo-Fetal Toxicity: May cause fetal harm. Advise females of reproductive potential to use effective contraception during treatment and for at least 5 months after the final dose.
5. Adverse Reactions
Most Common:
Fatigue, musculoskeletal pain, rash, diarrhea, nausea, cough, dyspnea, and pruritus.
Serious Adverse Events:
Immune-mediated pneumonitis, colitis, hepatitis, nephritis, and endocrinopathies (e.g., hypothyroidism, thyroiditis).
6. Drug Interactions
Immunosuppressants:
Avoid use of systemic corticosteroids and other immunosuppressants at baseline unless necessary for controlling immune-mediated adverse reactions.
Metabolic Enzymes:
Nivolumab is not metabolized by CYP enzymes; drug interactions involving CYP450 are not expected.
