1. Indications and Usage
Type 2 Diabetes Mellitus:
Indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
Combination Therapy:
Can be used as monotherapy or in combination with other antidiabetic agents (e.g., Metformin) when glycemic control is not achieved with diet, exercise, or single-agent therapy.
2. Dosage and Administration
Route of Administration:
For Oral Use ONLY.
Recommended Dosage:
The recommended dose is 5 mg once daily.
Administration Instructions:
Can be taken with or without food, as absorption is not significantly affected by meals.
The tablet should be swallowed whole with water.
Special Populations:
Renal Impairment: No dosage adjustment is required for patients with mild to moderate renal impairment. Caution is advised in severe renal impairment, although specific dosing data may vary by region.
Elderly: No dosage adjustment is required solely based on age, but monitoring of glycemic control is recommended.
3. Mechanism of Action
DPP-4 Inhibition:
Evogliptin is a potent, selective, and reversible inhibitor of the dipeptidyl peptidase-4 (DPP-4) enzyme.
Glucoregulatory Hormone Modulation:
By inhibiting DPP-4, it prevents the degradation of incretin hormones, specifically glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP).
Glucose-Dependent Insulin Secretion:
Elevated levels of active GLP-1 and GIP stimulate insulin release from pancreatic beta cells in a glucose-dependent manner. Simultaneously, they suppress glucagon secretion from alpha cells, thereby reducing hepatic glucose production and improving overall glycemic control.
4. Safety and Warnings
Hypersensitivity Reactions:
Hypersensitivity reactions (e.g., angioedema, urticaria, skin eruptions) have been reported. Discontinue immediately if such reactions occur.
Pancreatitis:
Acute pancreatitis has been reported in post-marketing experience with DPP-4 inhibitors. Patients should be monitored for persistent severe abdominal pain.
Renal Function:
Although generally safe, periodic monitoring of renal function is recommended, especially in elderly patients or those with pre-existing renal impairment.
5. Adverse Reactions
Most Common:
The drug is generally well-tolerated. Reported adverse reactions include nasopharyngitis, headache, nausea, and hyperglycemia.
Hypoglycemia:
When used as monotherapy, the incidence of hypoglycemia is low. However, the risk increases when combined with insulin secretagogues (e.g., sulfonylureas) or insulin。
6. Drug Interactions
Metabolic Pathway:
Evogliptin is minimally metabolized by CYP450 enzymes and is not a significant inhibitor or inducer of these enzymes.
Clinical Significance:
No clinically significant drug-drug interactions have been established. It can be safely co-administered with metformin, sulfonylureas, or thiazolidinediones with standard monitoring.
