\nIndications and Usage<\/p>\n
Letermovir is indicated for:<\/p>\n
Prevention of cytomegalovirus (CMV) infection and disease in \u200cadult and pediatric patients 6 months of age and older weighing at least 6kg\u200c who are CMV seropositive [R+] recipients of allogeneic hematopoietic stem cell transplant (HSCT).
\nPrevention of CMV disease in \u200cadult and pediatric patients 12 years of age and older weighing at least 40kg\u200c who are high-risk kidney transplant recipients (donor CMV seropositive \/ recipient CMV seronegative [D+\/R-]).
\nDosage and Administration
\nFor patients 12 years of age and older weighing \u2265 30kg (HSCT) or \u2265 40kg (kidney transplant):
\nStandard dose\u200c: 480 mg orally once daily
\nConcomitant use with cyclosporine\u200c: Reduce dose to \u200c240 mg orally once daily\u200c
\nDuration\u200c:
\nHSCT: Continue dosing until 100 days after transplant, can extend to 200 days for high-risk patients
\nKidney transplant: Continue dosing until 200 days after transplant
\nFor pediatric patients 6 months to 12 years old, or 12 years and older weighing < 30kg (HSCT):
\n15kg \u2264 weight < 30kg: 240 mg once daily
\n7.5kg \u2264 weight < 15kg: 120 mg once daily
\n6kg \u2264 weight < 7.5kg: 80 mg once daily
\nUse of oral pellets is required for patients weighing less than 30kg, tablet use is not recommended
\nAll doses are given once daily starting between Day 0 and Day 28 after transplant, continuing until Day 100 after transplant, can extend to 200 days for high-risk patients
\nLetermovir tablets can be taken with or without food.
\nContraindications<\/p>\n
Letermovir is contraindicated for concomitant use with:<\/p>\n
Pimozide
\nErgot alkaloids (ergotamine, dihydroergotamine)
\nPitavastatin or simvastatin when combined with cyclosporine
\nWarnings and Precautions<\/p>\n
