1. Indications and Usage
\nColorectal Cancer:
\nIndicated for the treatment of patients with EGFR-expressing, metastatic colorectal cancer as a monotherapy or in combination with irinotecan.
\nHead and Neck Cancer:
\nLocally Advanced: Indicated in combination with radiation therapy for the treatment of locally advanced squamous cell carcinoma of the head and neck.
\nRecurrent\/Metastatic: Indicated in combination with platinum-based therapy for the treatment of recurrent or metastatic squamous cell carcinoma of the head and neck.
\n2. Dosage and Administration
\nRecommended Dosage:
\nLoading Dose: 400 mg\/m\u00b2 administered as an initial 120-minute intravenous infusion.
\nMaintenance Dose: 250 mg\/m\u00b2 administered as a 60-minute intravenous infusion once weekly thereafter.
\nPre-Medication:
\nAdminister an antihistamine (e.g., diphenhydramine 25\u201350 mg IV) 1 hour before the first infusion.
\nAdministration:
\nDo not administer as an intravenous push or bolus. Use a non-PVC infusion set.
\n3. Mechanism of Action
\nEGFR Inhibition:
\nCetuximab binds to the extracellular domain of the epidermal growth factor receptor (EGFR) with higher affinity than the natural ligands.
\nPhysiological Effect:
\nReceptor occupancy prevents phosphorylation and activation of receptor-associated kinases, thereby inhibiting cell growth, inducing apoptosis, decreasing metastasis and angiogenesis, and increasing tumor cell sensitivity to the cytotoxic effects of radiation and some chemotherapeutic agents.
\n4. Safety and Warnings
\nInfusion-Related Reactions:
\nSevere, potentially fatal reactions (e.g., acute pulmonary edema, hypotension, bronchospasm) can occur. Up to 90% occur during the first infusion. Monitor patients closely during and for at least 1 hour after infusion.
\nPulmonary Toxicity:
\nInterstitial lung disease (ILD) has been reported. Discontinue cetuximab in patients with severe or progressive pulmonary symptoms.
\nDermatologic Toxicity:
\nAcneiform rash is very common and correlates with efficacy. Severe skin reactions (e.g., necrosis, bullous eruptions) may occur.
\nEmbryo-Fetal Toxicity:
\nCan cause fetal harm. Advise females of reproductive potential to use effective contraception.
\n5. Adverse Reactions
\nMost Common:
\nAcneiform rash, fatigue, infection, diarrhea, nausea, vomiting, anorexia, and headache.
\nElectrolyte Disturbances:
\nHypomagnesemia occurs frequently due to renal magnesium wasting. Monitor serum magnesium levels.
\n6. Drug Interactions
\nRadiation Therapy:
\nConcomitant use with radiation therapy has been associated with increased cardiopulmonary toxicity (cardiac arrest, cardiopulmonary arrest).
\nChemotherapy:
\nPharmacokinetic interactions with irinotecan or platinum agents have not been definitively established, but clinical monitoring is required.
