1. Indications and Usage
\nVon Hippel-Lindau (VHL) Disease:
\nIndicated for the treatment of adult and pediatric patients (12 years and older) with VHL disease requiring therapeutic intervention for renal cell carcinoma (RCC), central nervous system (CNS) hemangioblastomas, or pancreatic neuroendocrine tumors (pNETs), not requiring immediate surgery.
\nAdvanced Renal Cell Carcinoma (RCC):
\nIndicated for the treatment of adult patients with advanced RCC who have been treated with prior anti-PD-1\/L1 inhibitor and anti-angiogenic therapy.
\n2. Dosage and Administration
\nRecommended Dosage:
\nDose: 120 mg orally once daily.
\nAdministration: Swallow tablets whole; do not crush or chew. Can be taken with or without food.
\nDose Modifications:
\nAnemia: Interrupt if Hemoglobin < 8 g\/dL; resume at reduced dose when \u2265 8 g\/dL.
\nHypoxia: Interrupt if resting oxygen saturation < 88%; resume at reduced dose when resolved.
\nMissed Dose:
\nIf a dose is missed, it may be taken as soon as possible on the same day. Do not take extra tablets to make up for the missed dose.
\n3. Mechanism of Action
\nHIF-2\u03b1 Inhibition:
\nBelzutifan is a small molecule inhibitor of hypoxia-inducible factor-2\u03b1 (HIF-2\u03b1).
\nPhysiological Effect:
\nIt binds to the HIF-2\u03b1 protein, blocking its interaction with HIF-1\u03b2. This inhibits the transcription and expression of HIF-2\u03b1 target genes involved in angiogenesis (e.g., VEGF), cell proliferation, and tumor growth.
\n4. Safety and Warnings
\nAnemia:
\nAnemia occurs frequently and can be severe. Monitor hemoglobin levels prior to treatment and regularly during treatment.
\nHypoxia:
\nHypoxia may occur, sometimes requiring oxygen supplementation or hospitalization. Monitor oxygen saturation levels.
\nEmbryo-Fetal Toxicity:
\nCan cause fetal harm. Verify pregnancy status before starting. Advise females of reproductive potential to use effective non-hormonal contraception during treatment and for 1 week after the final dose.
\n5. Adverse Reactions
\nMost Common (\u226520%):
\nAnemia, fatigue, headache, dizziness, hypoxia, diarrhea, nausea, musculoskeletal pain, elevated creatinine, hyperglycemia, and cough.
\n6. Drug Interactions
\nUGT2B17 or CYP2C19 Inhibitors:
\nMay increase belzutifan exposure. Monitor for anemia and hypoxia and consider dose reduction.
\nHormonal Contraceptives:
\nMay reduce the efficacy of hormonal contraceptives. Use non-hormonal methods.
