1. Indications and Usage
\nParoxysmal Nocturnal Hemoglobinuria (PNH):
\nIndicated for the treatment of PNH to reduce hemolysis.
\nAtypical Hemolytic Uremic Syndrome (aHUS):
\nIndicated for the treatment of aHUS to inhibit complement-mediated thrombotic microangiopathy (TMA).
\nGeneralized Myasthenia Gravis (gMG):
\nIndicated for the treatment of adult patients who are anti-acetylcholine receptor (AchR) antibody-positive.
\nNeuromyelitis Optica Spectrum Disorder (NMOSD):
\nIndicated for the treatment of adult patients who are anti-aquaporin-4 (AQP4) antibody-positive.
\n2. Dosage and Administration
\nRoute of Administration:
\nIntravenous (IV) Infusion ONLY. Do not administer as an IV push or bolus.
\nAdult Dosage (PNH):
\nLoading: 600 mg weekly for the first 4 weeks.
\nMaintenance: 900 mg on Week 5, then 900 mg every 2 weeks thereafter.
\nAdult Dosage (aHUS \/ gMG \/ NMOSD):
\nLoading: 900 mg weekly for the first 4 weeks.
\nMaintenance: 1200 mg on Week 5, then 1200 mg every 2 weeks thereafter.
\nPre-treatment Requirement:
\nPatients must be vaccinated against Neisseria meningitidis at least 2 weeks prior to the first dose.
\n3. Mechanism of Action
\nComplement C5 Inhibition:
\nEculizumab is a humanized monoclonal antibody that binds with high affinity to the complement protein C5.
\nPhysiological Effect:
\nIt inhibits the cleavage of C5 into C5a and C5b, thereby preventing the formation of the terminal complement complex C5b-9 (Membrane Attack Complex).
\n4. Safety and Warnings
\nSerious Meningococcal Infections (BLACK BOX WARNING):
\nEculizumab increases the risk of systemic meningococcal infections, which can be fatal if not recognized early.
\nInfusion Reactions:
\nMay occur during or within 24 hours of infusion. Monitor for symptoms like headache, hypertension, or dyspnea.
\nVaccination Considerations:
\nVaccination may further activate complement; monitor for disease exacerbation after vaccination.
\nAdverse Reactions
\nMost Common:
\nHeadache, nasopharyngitis, diarrhea, nausea, fatigue, and hypertension (specifically in aHUS).
\nInfections:
\nIncreased susceptibility to infections, particularly encapsulated bacteria (e.g., Streptococcus pneumoniae, Haemophilus influenzae).
\n6. Drug Interactions
\nLive Vaccines:
\nAvoid the use of live bacterial or viral vaccines during treatment.
\nNo Significant Metabolic Interactions:
\nAs a monoclonal antibody, it does not interact with CYP450 enzymes.
