1. Indications and Usage
\nMetastatic Small Cell Lung Cancer (SCLC):
\nIndicated for the treatment of adult patients with metastatic small cell lung cancer (SCLC) who have disease progression during or after platinum-containing chemotherapy.
\nNote: This indication is approved under accelerated approval based on objective response rate and duration of response. Continued approval may be contingent upon verification and description of clinical benefit in confirmatory trials.
\n2. Dosage and Administration
\nRoute of Administration:
\nIntravenous Infusion ONLY. Do not administer as an IV push or bolus.
\nRecommended Dosage:
\nDose: 3.2 mg\/m\u00b2 administered intravenously over 1 hour.
\nFrequency: Once every 21 days until disease progression or unacceptable toxicity.
\nPrerequisites: Ensure Absolute Neutrophil Count (ANC) \u2265 1,500\/mm\u00b3 and Platelet Count \u2265 100,000\/mm\u00b3 before starting treatment.
\nPremedication:
\nConsider administering antiemetics (e.g., Dexamethasone 8 mg IV or equivalent + Ondansetron 8 mg IV or equivalent) prior to each infusion to prevent nausea and vomiting.
\n3. Mechanism of Action
\nDNA Alkylation and Transcription Inhibition:
\nLurbinectedin binds to guanine residues in the DNA minor groove, forming adducts that cause bending of the DNA helix. This triggers a cascade of events affecting DNA-binding proteins (including certain transcription factors) and DNA repair pathways, leading to cell cycle arrest and apoptosis.
\nRNA Polymerase II Inhibition:
\nIt inhibits RNA polymerase II activity, blocking protein synthesis in tumor cells.
\n4. Safety and Warnings
\nMyelosuppression:
\nCan cause severe neutropenia, thrombocytopenia, and anemia. Monitor complete blood counts (CBC) prior to each dose.
\nEmbryo-Fetal Toxicity:
\nMay cause fetal harm. Advise females and males of reproductive potential to use effective contraception during treatment and for at least 6 months (females) or 4 months (males) after the final dose.
\nHepatotoxicity:
\nMonitor liver function tests (ALT\/AST) regularly.
\nRhabdomyolysis:
\nMonitor for signs of muscle pain or dark urine.
\n5. Adverse Reactions
\nMost Common:
\nNeutropenia, fatigue, anemia, nausea, decreased appetite, constipation, vomiting, increased creatinine, increased ALT\/AST, pyrexia, diarrhea, and cough.
\n6. Drug Interactions
\nCYP3A4 Modulators:
\nLurbinectedin is metabolized by CYP3A4. Avoid concomitant use with strong CYP3A4 inhibitors or inducers.
\nDietary Restrictions:
\nAvoid grapefruit and grapefruit juice as they inhibit CYP3A4 and may increase drug exposure.
