1. Indications and Usage
\nCholesterol Gallstones:
\nIndicated for the dissolution of cholesterol gallstones in patients with radiolucent, functioning gallbladders who are not candidates for surgery.
\nPrimary Biliary Cholangitis (PBC):
\nIndicated for the treatment of primary biliary cholangitis (formerly primary biliary cirrhosis).
\nCystic Fibrosis-Associated Liver Disease (CF-LD):
\nIndicated for the treatment of liver disease associated with cystic fibrosis.
\nBile Reflux Gastritis:
\nIndicated for the treatment of bile reflux gastritis.
\n2. Dosage and Administration
\nRoute of Administration:
\nOral Use ONLY. Shake well before use.
\nRecommended Dosage:
\nCholesterol Gallstones: 8\u201310 mg\/kg\/day, taken at bedtime (with or without food).
\nPrimary Biliary Cholangitis (PBC): 13\u201315 mg\/kg\/day, administered in 2\u20134 divided doses.
\nCystic Fibrosis-Associated Liver Disease: 20\u201330 mg\/kg\/day, administered in 2\u20134 divided doses.
\nBile Reflux Gastritis: 250 mg once daily at bedtime.
\nAdministration Instructions:
\nUse the supplied measuring device (syringe or spoon) to ensure accurate dosing.
\nSuspension can be diluted with water or juice if the taste is unacceptable.
\n3. Mechanism of Action
\nCholeresis:
\nUrsodeoxycholic acid (UDCA) is a hydrophilic bile acid that stimulates bile secretion (choleresis) by the liver.
\nInhibition of Cholesterol Absorption:
\nIt reduces the absorption of cholesterol from the intestine and decreases the secretion of cholesterol into the bile, thereby reducing the cholesterol saturation index in bile.
\nCytoprotection:
\nIt replaces toxic, hydrophobic bile acids in the biliary tree and protects hepatocytes from bile acid-induced cytotoxicity.
\n4. Safety and Warnings
\nContraindications:
\nAcute inflammation of the gallbladder or bile ducts.
\nRadiopaque (calcified) gallstones.
\nNon-functioning gallbladder.
\nSevere hepatic impairment or biliary obstruction.
\nMonitoring:
\nMonitor liver function tests (ALT, AST, GGT) at regular intervals.
\nMonitor gallstone status via ultrasound every 6 months.
\n5. Adverse Reactions
\nMost Common:
\nDiarrhea, nausea, vomiting, constipation, and pruritus.
\n6. Drug Interactions
\nCholesterol-Lowering Drugs:
\nDrugs like colestyramine, colestipol, and aluminum-based antacids may bind to UDCA in the gut and prevent its absorption. Avoid concomitant use.
\nCyclosporine:
\nUDCA may increase the intestinal absorption of cyclosporine. Monitor cyclosporine levels.
