1. Indications and Usage
\nPulmonary Arterial Hypertension (PAH):
\nIndicated for the treatment of PAH (WHO Group 1) to reduce symptoms associated with exercise.
\nApproved for use in patients with PAH associated with congenital heart disease.
\nRoute of Administration:
\nAdministered via continuous subcutaneous (SC) or intravenous (IV) infusion.
\n2. Dosage and Administration
\nInitial Dosage:
\nThe recommended starting dose is 1.25 ng\/kg\/min (either SC or IV).
\nFor patients who cannot tolerate this initial dose due to systemic effects (e.g., headache, nausea), the rate may be reduced to 0.625 ng\/kg\/min.
\nTitration:
\nIncrease the infusion rate in small increments (e.g., 1.25 ng\/kg\/min for the first 4 weeks, then 2.5 ng\/kg\/min thereafter) based on clinical response and tolerability.
\nAdministration Instructions:
\nSubcutaneous: Preferred route. Administered via a dedicated pump using a catheter inserted subcutaneously (usually the abdominal wall).
\nIntravenous: Administered via a central venous catheter. Peripheral IV use is limited to short-term due to the risk of thrombophlebitis.
\nDilution: If dilution is required, use Sterile Water for Injection or 0.9% Sodium Chloride Injection.
\nMissed Dose\/Interruption:
\nDo not abruptly stop the infusion.
\nIf the infusion is interrupted for a short period (hours), resume at the same rate.
\nIf interruption is prolonged, re-titration of the dose is required.
\n3. Mechanism of Action
\nProstacyclin Analogue:
\nTreprostinil is a potent, long-acting, stable analogue of endogenous prostacyclin (PGI2).
\nVasodilation:
\nIt directly dilates pulmonary and systemic arterial vascular beds.
\nAntiplatelet Effects:
\nIt inhibits platelet aggregation.
\nPathophysiology:
\nIn PAH, there is a deficiency of endogenous prostacyclin. Treprostinil replaces this deficiency, reducing pulmonary vascular resistance and right ventricular afterload.
\n4. Safety and Warnings
\nSystemic Hypotension:
\nDue to its vasodilatory properties, sudden withdrawal or rapid dose reduction may lead to rebound pulmonary hypertension or hypotension.
\nInfusion Site Complications:
\nSC: Pain and site reactions (rash, erythema, induration) are very common and may limit dose escalation.
\nIV: Risk of bloodstream infections and sepsis. Strict aseptic technique is mandatory.
\nBleeding Risk:
\nTreprostinil inhibits platelet aggregation, which may increase the risk of bleeding, especially in patients on anticoagulation therapy.
\nFluid Retention:
\nMay cause fluid retention, which can be problematic in patients with right ventricular failure.
\n5. Adverse Reactions
\nMost Common (SC):
\nInjection site pain (very common), injection site reaction, headache, diarrhea, nausea, jaw pain, and flushing.
\nMost Common (IV):
\nHeadache, diarrhea, nausea, vomiting, and infection at the catheter site.
\n6. Drug Interactions
\nAntihypertensives\/Other Vasodilators:
\nConcomitant use with diuretics, antihypertensive drugs, or other vasodilators may increase the risk of symptomatic hypotension.
\nAnticoagulants\/Antiplatelets:
\nConcomitant use may increase the risk of bleeding.
\nCYP Enzymes:
\nTreprostinil is metabolized primarily by CYP2C8. Co-administration with CYP2C8 inhibitors (e.g., gemfibrozil) may increase exposure, while inducers (e.g., rifampin) may decrease it.
