\nHemophilia A (Congenital Factor VIII Deficiency):
\nIndicated for control and prevention of bleeding episodes in adults and children with Hemophilia A (congenital Factor VIII deficiency).
\nSurgical Prophylaxis:
\nIndicated for perioperative management in adults and children with Hemophilia A.
\nNote: Not indicated for von Willebrand Disease.
\n2. Dosage and Administration
\nRoute of Administration:
\nFor Intravenous (IV) Injection ONLY.
\nDosing Principles:
\nDosage and duration of treatment depend on the severity of Factor VIII deficiency, the location and extent of bleeding, and the patient’s clinical condition.
\nFormula: 1 International Unit (IU) of Factor VIII per kg body weight typically raises the circulating Factor VIII activity by approximately 2% of normal activity.
\nRecommended Dosage:
\nMinor Bleeding: 20\u201340 IU\/kg every 12\u201324 hours.
\nModerate to Severe Bleeding: 40\u201360 IU\/kg every 8\u201324 hours.
\nSevere Hemorrhage: 60\u2013100 IU\/kg every 8\u201312 hours.
\nSurgical Prophylaxis:
\nMinor surgery: 25\u201340 IU\/kg.
\nMajor surgery: 50\u2013100 IU\/kg (aiming for 80\u2013100% activity).
\n3. Mechanism of Action
\nCoagulation Replacement:
\nOctocog alfa is a recombinant form of human coagulation Factor VIII.
\nClotting Cascade:
\nIn the presence of Calcium ions (Ca\u00b2\u207a), activated Factor VIII (Factor VIIIa) acts as a cofactor for activated Factor IX (Factor IXa). This complex accelerates the conversion of Factor X to activated Factor X (Factor Xa), which subsequently leads to the generation of thrombin and the formation of a fibrin clot.
\n4. Safety and Warnings
\nInhibitor Formation:
\nThe most important safety concern is the development of neutralizing antibodies (inhibitors) against Factor VIII. Patients must be monitored for the development of inhibitors.
\nHypersensitivity\/Allergic Reactions:
\nHypersensitivity reactions (including anaphylaxis) may occur. If signs of allergy appear, discontinue administration immediately.
\nThrombotic Events:
\nAlthough rare with Factor VIII, thrombotic events can occur, particularly in patients with cardiovascular risk factors.

\nPyrexia (fever), headache, rash, and injection site reactions (burning\/stinging).
