1. Indications and Usage
\nAnemia Due to Chronic Kidney Disease (CKD):
\nIndicated for the treatment of anemia associated with CKD in adult and pediatric patients.
\nGoal: To reduce the need for red blood cell (RBC) transfusions.
\nAnemia Due to Chemotherapy:
\nIndicated for the treatment of anemia in adult patients with non-myeloid malignancies where anemia is attributable to concurrent chemotherapy.
\nLimitation: Therapy is limited to patients with a planned chemotherapy duration of at least 2 months.
\n2. Dosage and Administration
\nRoute of Administration:
\nFor Intravenous (IV) or Subcutaneous (SC) Injection.
\nDosage Principles:
\nDosage must be individualized based on the patient’s response.
\nRecommended Dosage (CKD):
\nStarting Dose:
\nHemodialysis Patients: 0.45 mcg\/kg once weekly IV or SC, or 0.75 mcg\/kg once every 2 weeks SC.
\nNon-Dialysis Patients: 0.45 mcg\/kg once weekly IV or SC.
\nDose Adjustment:
\nAdjust the dose to maintain Hb levels within the target range (typically 10\u201311 g\/dL).
\nIf Hb increases >1 g\/dL (10 g\/L) in any 2-week period, reduce the dose by at least 25%.
\nWithhold dose if Hb exceeds target range to avoid risks of thrombosis.
\n3. Mechanism of Action
\nEPO Receptor Agonist:
\nDarbepoetin alfa is a glycosylated analog of endogenous erythropoietin (EPO).
\nEnhanced Half-Life:
\nIt contains 5 N-linked carbohydrate chains (compared to 3 in native EPO), which increases its molecular weight and serum half-life (approx. 3 times longer than epoetin alfa).
\nErythropoiesis:
\nIt binds to EPO receptors on bone marrow cells, stimulating the proliferation and differentiation of erythroid progenitor cells into red blood cells.
\n4. Safety and Warnings
\nIncreased Risk of Mortality and Thrombosis:
\nESAs increase the risk of death, myocardial infarction, stroke, venous thromboembolism, and thrombosis of vascular access.
\nBlack Box Warning: Safety is reduced when targeting Hb >11 g\/dL.
\nTumor Progression:
\nIn patients with malignancy, ESAs may shorten overall survival and\/or increase the risk of tumor progression or recurrence.
\nSevere Skin Reactions:
\nSerious skin reactions, including Stevens-Johnson syndrome and toxic epidermal necrolysis, have been reported.
\nPure Red Cell Aplasia (PRCA):
\nRare cases of PRCA have occurred with ESA therapy.
\n5. Adverse Reactions
\nMost Common:
\nHypertension, headache, arthralgia, nausea, edema, fatigue, and vomiting.
\nSerious Events:
\nStroke, myocardial infarction, seizure, and thromboembolic events.
\n6. Drug Interactions

\nNo formal drug-drug interaction studies have been conducted.
\nConcomitant Use:
\nUse caution with drugs that may interact with the ESA pathway or affect hemoglobin levels.
