1. Indications and Usage
\nAnemia in Non-Transfusion-Dependent \u03b2-Thalassemia (NTDT):
\nIndicated for the treatment of anemia in adult patients with beta thalassemia who require regular red blood cell (RBC) transfusions.
\nAnemia in Myelodysplastic Syndromes (MDS):
\nIndicated for the treatment of anemia in adult patients with lower-risk myelodysplastic syndromes with ring sideroblasts (MDS-RS) who have an inadequate response to ESAs and who require RBC transfusions.
\n2. Dosage and Administration
\nRoute of Administration:
\nFor Subcutaneous Injection ONLY.
\nReconstitution:
\nReconstitute the lyophilized powder in Sterile Water for Injection. Do not shake or vigorously swirl.
\nRecommended Dosage (NTDT):
\nStarting Dose: 1 mg\/kg once every 3 weeks.
\nDose Adjustment: If transfusion burden does not decrease after at least 2 doses (6 weeks), increase to 1.25 mg\/kg.
\nMaximum Dose: 1.25 mg\/kg once every 3 weeks.
\nRecommended Dosage (MDS-RS):
\nStarting Dose: 1 mg\/kg once every 3 weeks.
\nDose Adjustment: If transfusion burden does not decrease after 2 doses, increase by increments (e.g., to 1.33 mg\/kg) up to a maximum of 1.75 mg\/kg.
\nDose Interruption:
\nWithhold dose if pre-dose hemoglobin is \u226511.5 g\/dL (in the absence of recent transfusion). Resume when Hb \u226411 g\/dL.
\n3. Mechanism of Action
\nActivin Receptor Ligand Trap:
\nLuspatercept is a recombinant fusion protein that binds to specific ligands in the transforming growth factor-beta (TGF-\u03b2) superfamily.
\nErythroid Maturation:
\nIt acts as a ligand trap to reduce SMAD2\/3 signaling. This promotes late-stage erythroid maturation, thereby improving ineffective erythropoiesis and increasing hemoglobin levels.
\n4. Safety and Warnings
\nHypertension:
\nHypertension and hypertensive crisis have been reported. Monitor blood pressure regularly and treat as clinically indicated.
\nHypersensitivity Reactions:
\nReactions may occur; patients should be observed for signs of anaphylaxis after administration.
\nEmbryo-Fetal Toxicity:
\nMay cause fetal harm based on its mechanism of action.
\n5. Adverse Reactions
\nMost Common:
\nArthralgia (joint pain), headache, hypertension, dizziness, diarrhea, and fatigue.
\nInjection Site Reactions:
\nPain, erythema, and bruising at the injection site.

\nConcomitant Use:
\nCaution is advised when using with other agents that may affect blood pressure or erythropoiesis.
