1. Indications and Usage
\nThrombocytopenia in Chronic Liver Disease (CLD):
\nIndicated for the treatment of thrombocytopenia in adult patients with chronic liver disease who are scheduled to undergo a procedure.
\nNote: Dosing is based on baseline platelet count.
\nImmune Thrombocytopenia (ITP):
\nIndicated for the treatment of previously treated chronic immune thrombocytopenia in adult patients who have an inadequate response to previous therapies.
\n2. Dosage and Administration
\nRoute of Administration:
\nFor Oral Use ONLY.
\nAdministration Instructions:
\nCan be taken with or without food (unlike Eltrombopag, it is not significantly affected by dietary calcium or polyvalent cations).
\nRecommended Dosage (CLD – Periprocedural):
\nBaseline Platelet Count < 40 \u00d7 10\u2079\/L: 60 mg once daily for 5 days.
\nBaseline Platelet Count 40\u2013< 50 \u00d7 10\u2079\/L: 40 mg once daily for 5 days.
\nProcedure Timing: The scheduled procedure should be performed 5 to 8 days after the last dose.
\nRecommended Dosage (ITP – Chronic):
\nStarting Dose: 20 mg once daily.
\nTitration: Adjust dose to maintain the lowest platelet count sufficient to reduce bleeding risk.
\nDiscontinuation: If platelet count is below normal range after 2 consecutive weeks of treatment, discontinue dosing.
\n3. Mechanism of Action
\nTPO Receptor Agonist:
\nAvatrombopag is an orally bioavailable small molecule that binds to the transmembrane domain of the human TPO receptor (c-Mpl).
\nNon-Competitive Binding:
\nUnlike endogenous TPO, it binds to a different site and does not compete with TPO for receptor binding.
\nPlatelet Production:
\nIt stimulates the proliferation and differentiation of megakaryocyte progenitor cells in the bone marrow, leading to increased platelet production.
\n4. Safety and Warnings
\nThrombotic\/Thromboembolic Complications:
\nTreatment may increase the risk of thrombotic\/thromboembolic complications (e.g., Portal Vein Thrombosis).
\nMonitor for signs of thrombosis.
\nDiscontinue Avatrombopag if platelet count exceeds the target range or rises too rapidly.
\nLoss of Response:
\nIf the response is lost (i.e., platelet count does not increase despite adherence), consider the development of neutralizing antibodies.
\n5. Adverse Reactions
\nMost Common (CLD):
\nHeadache, nausea, diarrhea, asthenia, and fever.
\nMost Common (ITP):
\nHeadache, fatigue, upper abdominal pain, and nasopharyngitis.
\nSerious Reactions:
\nPortal vein thrombosis, hepatic encephalopathy.
\n6. Drug Interactions
\nCYP Enzymes:
\nAvatrombopag is a substrate of CYP2C9 and CYP3A4.
\nInhibitors: Concomitant use with moderate\/strong CYP2C9\/CYP3A4 inhibitors may increase Avatrombopag exposure (reduce dose in ITP).
\nInducers: Concomitant use with moderate\/strong CYP2C9\/CYP3A4 inducers may decrease Avatrombopag exposure (increase dose in ITP).
\nNote: In CLD patients, dose adjustments for drug interactions are generally not required due to the short-term treatment course.
