1. Indications and Usage
\nMetastatic Colorectal Cancer (MCRC):
\nIn combination with intravenous 5-fluorouracil-based chemotherapy for first-line treatment.
\nNon-Squamous Non-Small Cell Lung Cancer (NSCLC):
\nIn combination with carboplatin and paclitaxel for unresectable, locally advanced, recurrent, or metastatic non-squamous NSCLC.
\nGlioblastoma (GBM):
\nAs a single agent for recurrent disease.
\nMetastatic Renal Cell Carcinoma (MRCC):
\nIn combination with interferon alfa.
\nCervical Cancer:
\nIn combination with paclitaxel and cisplatin or topotecan for persistent, recurrent, or metastatic disease.
\nOvarian Cancer:
\nIn combination with chemotherapy for advanced disease (specific indications vary by region and study).
\n2. Dosage and Administration
\nRoute of Administration:
\nFor Intravenous (IV) Infusion ONLY. Do not administer as an IV push or bolus.
\nRecommended Dosage:
\nMCRC: 5 mg\/kg every 2 weeks OR 7.5 mg\/kg every 3 weeks.
\nNSCLC, GBM, MRCC, Cervical Cancer: 15 mg\/kg every 3 weeks.
\nAdministration Instructions:
\nDilute the appropriate dose in 100 mL of 0.9% Sodium Chloride Injection.
\nFirst Infusion: Administer over 90 minutes.
\nSubsequent Infusions: If the first is well tolerated, the second can be over 60 minutes; if the 60-minute infusion is well tolerated, all subsequent infusions may be administered over 30 minutes.
\nContraindicated Diluents:
\nDo not dilute or administer with dextrose or glucose solutions.
\n3. Mechanism of Action
\nVEGF Inhibition:
\nBevacizumab binds to VEGF (Vascular Endothelial Growth Factor), preventing its interaction with VEGF receptors (Flt-1 and KDR) on the surface of endothelial cells.
\nAnti-Angiogenesis:
\nBy blocking VEGF signaling, it inhibits the proliferation and migration of endothelial cells, thereby preventing the formation of new blood vessels (angiogenesis) that tumors need to grow and metastasize.
\n4. Safety and Warnings
\nGastrointestinal Perforation:
\nCan occur at any time. Permanently discontinue if perforation occurs.
\nSurgery and Wound Healing Complications:
\nDo not administer for at least 28 days following elective surgery and do not administer until the wound is fully healed.
\nHemorrhage:
\nSevere or fatal hemorrhage, including hemoptysis (lung bleeding), may occur. Use with caution in patients with recent hemoptysis or histology showing squamous cell carcinoma.
\nArterial Thromboembolism:
\nIncreased risk of fatal arterial thromboembolic events (e.g., stroke, myocardial infarction).
\nHypertension and Proteinuria:
\nMonitor blood pressure and urinalysis regularly. Dose interruption or discontinuation is required for severe hypertension or nephrotic syndrome.
\nReversible Posterior Leukoencephalopathy Syndrome (RPLS):
\nA rare neurological disorder. Permanently discontinue if RPLS occurs.
\n5. Adverse Reactions
\nMost Common (>10%):
\nHypertension, fatigue or asthenia, diarrhea, abdominal pain, headache, and elevated transaminases.
\n6. Drug Interactions
\nChemotherapy Agents:
\nExtensively studied in combination with fluoropyrimidines, taxanes, and platinum agents. No significant pharmacokinetic interactions were observed.
