1. Indications and Usage
\nEarly Breast Cancer (EBC):
\nIndicated for the treatment of adult patients with HER2-positive early breast cancer.
\nNeoadjuvant Setting: In combination with chemotherapy for patients with tumors >2 cm or node-positive disease.
\nAdjuvant Setting: For patients with high-risk HER2-positive early breast cancer.
\nMetastatic Breast Cancer (MBC):
\nIndicated for adult patients with HER2-positive metastatic breast cancer who have not received prior anti-HER2 therapy or chemotherapy for metastatic disease, to be used in combination with docetaxel.
\n2. Dosage and Administration
\nRoute of Administration:
\nFor Subcutaneous Injection (SC) ONLY. Do not administer intravenously (IV).
\nRecommended Dosage:
\nLoading Dose: Pertuzumab 1200 mg + Trastuzumab 600 mg administered once every 3 weeks.
\nMaintenance Dose: Pertuzumab 600 mg + Trastuzumab 600 mg administered once every 3 weeks.
\nAdministration Instructions:
\nAdminister as a subcutaneous injection into the thigh over approximately 5 to 8 minutes.
\nInject into alternating thighs for subsequent doses.
\nDo not administer into areas where the skin is reddened, bruised, tender, or indurated.
\nMissed Dose:
\nIf the time between doses is <6 weeks, administer the maintenance dose as soon as possible.
\nIf the time between doses is \u22656 weeks, re-administer the loading dose.
\n3. Mechanism of Action
\nDual HER2 Blockade:
\nPertuzumab and Trastuzumab are both recombinant humanized IgG1 monoclonal antibodies that bind to the extracellular domain of the HER2 receptor, but at different epitopes (Domain II and Domain IV, respectively).
\nSynergistic Inhibition:
\nTrastuzumab: Primarily inhibits downstream signaling (PI3K\/Akt) and mediates antibody-dependent cellular cytotoxicity (ADCC).
\nPertuzumab: Primarily inhibits HER2 dimerization (specifically HER2-HER3 heterodimerization).
\nThe combination results in more complete blockade of HER2 signaling pathways and enhanced immune-mediated tumor cell killing compared to either antibody alone.
\n4. Safety and Warnings
\nCardiotoxicity (Left Ventricular Dysfunction):
\nCan cause a decrease in left ventricular ejection fraction (LVEF). Monitor LVEF prior to initiation and at regular intervals during treatment. Discontinue if significant LVEF decline occurs.
\nEmbryo-Fetal Toxicity:
\nCan cause fetal harm. Pregnancy testing is recommended before initiating treatment. Females of reproductive potential must use effective contraception during treatment and for at least 7 months after the final dose.
\nInfusion-Related Reactions:
\nHypersensitivity reactions can occur. Monitor patients during administration.
\n5. Adverse Reactions
\nMost Common:
\nNausea, diarrhea, rash, fatigue, vomiting, neutropenia, leukopenia, and anemia.
\nSpecific to Subcutaneous Formulation:
\nInjection site reactions (e.g., erythema, swelling) are common.
\n6. Drug Interactions
\nAnthracyclines:
\nConcomitant use with anthracyclines (e.g., doxorubicin, epirubicin) is generally not recommended due to the increased risk of cardiotoxicity, particularly in the adjuvant setting.
\nCarcinogenicity:
\nLong-term animal studies have not been performed to evaluate the carcinogenic or mutagenic potential of the combination.
