\nSchizophrenia:
\nTreatment of schizophrenia in adults.

\n2. Dosage and Administration
\nRoute of Administration:
\nFor Intramuscular (IM) Injection ONLY.
\nAdministered by a healthcare professional into the deltoid or gluteal muscle.
\nRecommended Dosage:
\nDosing Frequency: Typically administered every 4 weeks (monthly).
\nStarting Dose (Induction):
\nDay 1: 234 mg (Deltoid).
\nDay 8: 156 mg (Deltoid).

\n3. Mechanism of Action
\nReceptor Profile:
\nPaliperidone is the active metabolite of Risperidone (9-hydroxyrisperidone). It is a selective monoaminergic antagonist with high affinity for Serotonin (5-HT2A) and Dopamine (D2) receptors.

\nUnlike the microsphere technology used in Aripiprazole (Aristada), Paliperidone Palmitate is formulated as nanocrystals suspended in an aqueous vehicle.
\nDissolution-Limited Release: Upon IM injection, the drug dissolves slowly into the surrounding tissue fluid due to its low water solubility, forming a “drug depot.”
\nHydrolysis: The ester bond is hydrolyzed to release the active drug (paliperidone) into the systemic circulation.
\nHalf-Life: The apparent half-life ranges from 25 to 49 days depending on the injection site.
\n4. Safety and Warnings
\nIncreased Mortality in Elderly with Dementia (Boxed Warning):
\nElderly patients with dementia-related psychosis treated with antipsychotic drugs are at an increased risk of death.
\nSuicidal Thoughts and Behaviors:
\nMonitor for the emergence of suicidal ideation and behavior.
\nInjection Site Reactions:
\nPain, erythema, and induration at the injection site are common.
\nQTc Prolongation:
\nPaliperidone causes a modest increase in the QTc interval. Caution is advised in patients with cardiac history or those taking other QT-prolonging drugs.
\nElevated Prolactin:
\nPaliperidone is associated with hyperprolactinemia, which may lead to galactorrhea, gynecomastia, and menstrual irregularities.
\n5.Adverse Reactions

\nInjection site pain, headache, anxiety, insomnia, upper abdominal pain, nausea, vomiting, and diarrhea.
\n6.Drug Interactions
\nCYP3A4 Inducers:
\nCo-administration with strong CYP3A4 inducers (e.g., carbamazepine) may decrease paliperidone plasma levels; dose adjustment may be necessary.
\nQT-Prolonging Drugs:
\nConcomitant use with other drugs that prolong the QTc interval is not recommended.
