1. Indications and Usage
The product is indicated for the treatment of perimenopausal syndrome caused by natural or postoperative menopause.
2. Dosage and Administration
Recommended Dosage: One tablet daily for 28 days constitutes one treatment course.
Administration: The first 14 days should consist of the estradiol-only tablets, followed by 14 days of the estradiol and dydrogesterone tablets. The course should be continued without interruption. Treatment should use the lowest effective dose for the shortest duration.
3. Mechanism of Action
Estradiol: Identical in chemical and biological characteristics to endogenous estradiol, it acts as a potent ovarian hormone participating in uterine and adnexal functions.
Dydrogesterone: Added to counteract estrogen-induced endometrial proliferation, thereby significantly reducing the risk of endometrial hyperplasia and carcinoma associated with unopposed estrogen therapy.
4. Safety and Warnings
Thromboembolism: Hormone replacement therapy increases the risk of venous thromboembolism (deep vein thrombosis, pulmonary embolism) and arterial thromboembolism.
Malignancies: The overall risk of breast cancer increases with the duration of hormone replacement therapy. The risk of endometrial hyperplasia and carcinoma is increased in women with an intact uterus using estrogen without progesterone.
Cardiovascular and Hepatic: Caution is required in patients with cardiovascular risk factors, active liver disease, or severe hepatic impairment.
5. Adverse Reactions and Clinical Research
Adverse Reactions: Common adverse reactions include headache, migraine, breast pain/tenderness, nausea, abdominal pain, breakthrough bleeding, and spotting.
Clinical Research: Clinical studies have demonstrated significant alleviation of vasomotor symptoms (e.g., hot flashes) and urogenital atrophy while maintaining endometrial safety through the sequential addition of dydrogesterone.
6. Drug Interactions
Enzyme Inducers: Drugs such as barbiturates, rifampin, and carbamazepine may increase the metabolism of the active components.
Anticoagulants: May reduce the anticoagulant effect of drugs like warfarin.
Tamoxifen: The efficacy of tamoxifen may be reduced.
