1. Indications and Usage
Infections: Treatment of infections of the respiratory tract, ear, eye, skin, skin structures, bone, urinary tract, and reproductive system caused by susceptible organisms.
Streptococcal Pharyngitis: Treatment of streptococcal pharyngitis for clinical efficacy and prevention of rheumatic fever.
2. Dosage and Administration
Adults: 250-500 mg every 6 hours. For uncomplicated cystitis, skin and skin structure infections, or streptococcal pharyngitis, 500 mg every 12 hours may be used.
Pediatrics: 25-50 mg/kg/day divided into doses every 6 hours. For skin and skin structure infections or streptococcal pharyngitis, 12.5-50 mg/kg every 12 hours may be used.
Renal Impairment: Dosage adjustment is required based on creatinine clearance.
Administration: May be taken without regard to meals.
3. Mechanism of Action
Bactericidal Activity: Cefalexin is a bactericidal agent that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs).
Spectrum: It demonstrates activity against many gram-positive and some gram-negative bacteria.
4. Safety and Warnings
Hypersensitivity: Severe and occasionally fatal hypersensitivity reactions have been reported. Cefalexin should not be used in patients with a history of immediate hypersensitivity to cephalosporins.
Pseudomembranous Colitis: Broad-spectrum antibiotics may cause pseudomembranous colitis; use with caution in patients with a history of gastrointestinal disease.
Renal Impairment: The drug is primarily excreted by the kidneys; caution is required in patients with renal failure.
Pregnancy: Use during pregnancy only if clearly necessary.

Common: Nausea, vomiting, diarrhea, and abdominal discomfort are the most frequently reported adverse reactions.
Serious: Hypersensitivity reactions, dizziness, and transient renal impairment have been reported.

6. Drug Interactions
Probenecid: Can delay renal excretion of cefalexin, increasing blood concentrations and half-life.
Cholestyramine: Concomitant use may reduce the average plasma concentration of cefalexin.
Warfarin: Cephalosporins may enhance the anticoagulant effect of warfarin.
