1. Indications and Usage
Monotherapy or Combination: Indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus. It can be used as monotherapy or in combination with metformin, sulfonylureas, or insulin.
Target: Used when glycemic control is not achieved with diet and exercise alone, or when other oral antidiabetic agents are insufficient.
2. Dosage and Administration
Starting Dose: The recommended starting dose is 4 mg once daily or 2 mg twice daily, taken without regard to meals.
Titration: The dose can be increased to a maximum of 8 mg once daily or 4 mg twice daily after 8 to 12 weeks if the glycemic response is inadequate.
Combination Therapy: When combined with a sulfonylurea, the sulfonylurea dose may need to be reduced to minimize hypoglycemia risk. Use with insulin is generally discouraged due to heart failure risk.
3. Mechanism of Action
PPAR-γ Activation: Rosiglitazone tartrate is a thiazolidinedione and acts as a ligand for peroxisome proliferator-activated receptor-gamma (PPAR-γ).
Insulin Sensitization: It enhances insulin sensitivity in skeletal muscle and adipose tissue, decreases hepatic glucose output, and improves peripheral glucose uptake and utilization.
4. Safety and Warnings
Fluid Retention and Heart Failure: May cause fluid retention, which can exacerbate or lead to congestive heart failure. Contraindicated in NYHA Class III or IV heart failure.
Hepatotoxicity: Liver enzyme elevations have been reported. Baseline liver function tests are recommended.
Fractures: Increased incidence of bone fractures, particularly in women, has been observed.
Macular Edema: Cases of diabetic macular edema with visual impairment have been reported.
5. Adverse Reactions and Clinical Research
Common Adverse Reactions: Edema, weight gain, upper respiratory tract infection, and pharyngitis.
Hypoglycemia: Low risk as monotherapy, but significantly increased when combined with insulin or sulfonylureas.
Other Adverse Effects: Anemia, dyspnea, and headache have been reported.
Clinical Findings: Clinical trials demonstrate significant reductions in HbA1c, fasting plasma glucose, and postprandial glucose.
6. Drug Interactions
CYP2C8 Inhibitors: Gemfibrozil (strong CYP2C8 inhibitor) significantly increases rosiglitazone exposure; dose should be limited to 4 mg daily.
CYP2C8 Inducers: Rifampin (strong inducer) decreases rosiglitazone exposure.
Hypoglycemic Agents: Concomitant use with sulfonylureas or insulin increases hypoglycemia risk.
Oral Contraceptives: May reduce the efficacy of ethinyl estradiol and levonorgestrel.
