1. Indications and Usage
Rheumatoid Arthritis: Indicated for the treatment of adult patients with moderately to severely active rheumatoid arthritis.
Alopecia Areata: Indicated for the treatment of adult patients with severe alopecia areata.
Atopic Dermatitis: Indicated for the treatment of adult patients with moderate to severe atopic dermatitis.
2. Dosage and Administration
Standard Dosing: The recommended dosage is 2 mg or 4 mg taken orally once daily, depending on the specific indication and patient risk factors.
Administration: Tablets can be taken with or without food at any time of day.
Renal and Hepatic Impairment: Dosage adjustment is required for patients with moderate to severe renal impairment or end-stage renal disease. It is not recommended for patients with severe hepatic impairment.
3. Mechanism of Action
JAK Inhibition: Baricitinib selectively inhibits Janus kinases (JAK1 and JAK2), which are involved in the signaling of multiple cytokines and growth factors.
Anti-inflammatory Effect: By blocking these pathways, it suppresses the inflammatory response and immune dysregulation associated with autoimmune diseases, thereby alleviating symptoms and slowing disease progression.
4. Safety and Warnings
Serious Infections: Patients are at increased risk for serious bacterial, fungal, and viral infections (including tuberculosis and herpes zoster). Active serious infections are a contraindication.
Malignancy: An increased risk of malignancy, including lymphoma and lung cancer, has been observed.
Cardiovascular and Thrombosis: There is a risk of major adverse cardiovascular events (MACE), thrombosis (including deep vein thrombosis and pulmonary embolism), and mortality, particularly in patients with cardiovascular risk factors.
Hematologic and Hepatic: Dose-dependent decreases in neutrophils and hemoglobin, as well as increases in liver enzymes, lipid levels, and creatine phosphokinase, may occur.
5. Adverse Reactions and Clinical Research
Adverse Reactions: Common adverse reactions include upper respiratory tract infection, headache, nasopharyngitis, elevated LDL cholesterol, and herpes infections.
Clinical Research: Clinical trials have demonstrated that baricitinib significantly improves clinical response rates, reduces disease activity, and improves quality of life across rheumatoid arthritis, alopecia areata, and atopic dermatitis populations.
6. Drug Interactions
Immunosuppressants: Concomitant use with other immunosuppressants (e.g., azathioprine, cyclosporine) or biologic agents may increase the risk of infection.
OAT3 Inhibitors: Strong inhibitors of organic anion transporter 3 (OAT3), such as probenecid, can increase baricitinib exposure and may require dosage adjustment.
Live Vaccines: Live or attenuated vaccines should be avoided during treatment.
