1. Indications and Usage
Major Depressive Disorder (MDD): Treatment of various types of depression, including depression with anxiety and reactive depression.
Panic Disorder: Treatment of panic disorder, with or without agoraphobia.
Social Anxiety Disorder: Treatment of social phobia/social anxiety disorder.
Obsessive-Compulsive Disorder (OCD): Treatment of obsessive-compulsive disorder.
2. Dosage and Administration
Route of Administration: Oral administration ONLY.
Recommended Dosage: The starting dose is typically 20 mg once daily.
Titration: The dose may be increased gradually (e.g., by 10 mg or 12.5 mg increments) at weekly intervals based on clinical response and tolerability. The maximum daily dose ranges from 50 mg to 60 mg depending on the specific indication.
Critical Administration Instructions:
Timing: Take once daily, preferably in the morning, before or after meals.
Swallowing: Tablets must be swallowed whole; do not crush, chew, or break due to enteric-coated sustained-release properties.
Discontinuation: Do not stop abruptly. Taper the dose gradually over at least 1 to 2 weeks to avoid discontinuation symptoms (e.g., dizziness, sensory disturbances, anxiety).
3. Mechanism of Action
Serotonin Reuptake Inhibition: Paroxetine selectively inhibits the reuptake of serotonin (5-HT) at the presynaptic neuronal membrane.
Neurotransmission Enhancement: This increases the concentration of 5-HT in the synaptic cleft, enhancing serotonergic neurotransmission, which underlies its antidepressant and anxiolytic effects.
4. Safety and Warnings
Suicidal Ideation: Antidepressants increase the risk of suicidal thinking and behavior in children, adolescents, and young adults (≤24 years).
Serotonin Syndrome: Risk increases when used with other serotonergic drugs (e.g., triptans, tricyclics, other SSRIs/SNRIs).
MAOI Interaction: Contraindicated with Monoamine Oxidase Inhibitors (MAOIs). At least 14 days must pass after stopping an MAOI before starting paroxetine, and at least 7 days (preferably 14) after stopping paroxetine before starting an MAOI.
Bleeding Risk: May increase the risk of bleeding, especially when used with aspirin, NSAIDs, or anticoagulants.
Pregnancy: Use during pregnancy, particularly in the first trimester, may increase the risk of congenital cardiac malformations.
5. Adverse Reactions and Clinical Research
Most Common Adverse Reactions: Nausea, constipation, dry mouth, somnolence, insomnia, dizziness, fatigue, and sexual dysfunction.
Serious Adverse Reactions: Serotonin syndrome, abnormal bleeding, hyponatremia, and neuroleptic malignant syndrome (NMS)-like reactions.
Clinical Research Highlights: Clinical trials have demonstrated significant efficacy in reducing depressive symptoms and anxiety compared to placebo across multiple anxiety and mood disorders.
6. Drug Interactions
CYP2D6 Inhibitors: Paroxetine is a strong inhibitor of CYP2D6. Concomitant use with CYP2D6 substrates (e.g., thioridazine, pimozide) is contraindicated due to the risk of serious adverse reactions.
Anticoagulants: Increased risk of bleeding when used with warfarin or other anticoagulants.
Alcohol: Patients should avoid alcohol during treatment.
