1. Indications and Usage
Primary Indication: This nutritional emulsion is indicated for enteral nutrition in patients with severe impairment of digestive and/or absorptive functions (for example, short bowel syndrome, pancreatic insufficiency, chronic inflammatory bowel disease, chemotherapy/radiation-induced enteritis, critically ill patients, and patients after long-term parenteral nutrition) who cannot tolerate macromolecular nutrients.
General Use: It is also applicable for patients with gastrointestinal function that can meet or be unwilling to eat enough conventional food to meet the body’s nutritional needs.
2. Dosage and Administration
Route: Oral administration or tube feeding (via gastric, duodenal, or jejunal tubes).
Dosage: The dosage should be determined based on the patient’s weight and nutritional status to fully meet nutritional requirements. The recommended daily dose for complete enteral nutrition is not less than 1500 mL.
Administration: When continuous tube feeding is used, the dose should be increased gradually. The initial infusion rate should be slow (e.g., 20 mL/hour), and it should be increased by 20 mL/hour daily according to the patient’s intestinal tolerance and target dosage, with a maximum infusion rate of 100 mL/hour.
Preparation: Shake well before use and administer at room temperature. It does not require dilution for patients adapted to full-concentration feeding.
3. Mechanism of Action
Nutritional Support: It provides pre-digested nutrients (short peptides and medium-chain triglycerides) that require minimal enzymatic digestion, allowing for rapid absorption in the remaining functional intestinal tract.
Metabolic Pathways: Medium-chain triglycerides (MCTs) are directly absorbed into the portal vein and metabolized in the liver for energy, bypassing the lymphatic system, which reduces the metabolic burden on the pancreas and liver. Short peptides facilitate the preservation of intestinal mucosal integrity.
4. Safety and Warnings
Administration Precautions: It must not be administered intravenously.
Infusion Control: When using gravity mode, the infusion speed must be carefully controlled. Accidental large-dose administration may pose fatal risks to lethargic or comatose patients.
Monitoring: Fluid balance should be monitored during use. Use with caution in patients with severe organ failure (e.g., severe hepatic or renal insufficiency) based on nitrogen tolerance.
Storage: Do not use if the packaging is damaged, swollen, or if the contents have coagulated. Once opened, it can be stored in a refrigerator for a maximum of 24 hours.
5. Adverse Reactions and Clinical Research
Adverse Reactions: If the administration rate is too fast or the dosage is excessive, gastrointestinal adverse reactions such as nausea, vomiting, or diarrhea may occur.
Clinical Research: Clinical data indicate that short peptide formulas improve nutritional status and tolerance in patients with compromised gastrointestinal function compared to standard polymeric formulas.
6. Drug Interactions
Concurrent Medications: Other drugs should not be mixed with this emulsion to avoid changes in physical and chemical stability.
Anticoagulants: Due to the presence of Vitamin K, caution is advised when used with coumarin anticoagulants; regular management of coagulation is recommended.
7. Pharmaceutical Information
Composition: It is a compound preparation consisting of whey protein hydrolysate (short peptides, accounting for 18% of total energy), medium-chain triglycerides (MCT, accounting for 51% of total fat), fish oil, carbohydrates, vitamins, minerals, and trace elements.
Appearance: It is a light grayish-brown to grayish-brown emulsion. It should be a uniform liquid with a thin layer of closed milk skin and sediment thickness at the bottom not exceeding 2 mm.
Storage: Store at 15-25°C in a closed container and do not freeze.
