1. Indications and Usage
Guselkumab injection is indicated for the treatment of adults with moderate to severe plaque psoriasis who are candidates for systemic therapy or phototherapy.
2. Dosage and Administration
Recommended Dosage: The recommended dosage is 100 mg administered by subcutaneous injection at week 0 and week 4, followed by every 8 weeks thereafter.
Administration: Avoid injecting into areas where psoriasis is present. Patients may be instructed to self-inject after proper training.
Monitoring: Patients who do not respond adequately by week 16 should be reassessed, and discontinuation should be considered.
3. Mechanism of Action
Guselkumab is a human monoclonal antibody that selectively targets the p19 subunit of interleukin-23 (IL-23). By binding to IL-23, it inhibits the differentiation and proliferation of T cells, thereby reducing the inflammatory response associated with plaque psoriasis.
4. Safety and Warnings
Infections: Guselkumab may increase the risk of infections. Patients with important clinically significant active infections, including active tuberculosis, should not initiate therapy.
Hypersensitivity: Serious hypersensitivity reactions have been reported.
Vaccinations: Live or live-attenuated vaccines should be avoided during treatment with guselkumab.
5. Adverse Reactions and Clinical Research
Common Adverse Reactions: The most common adverse reactions include upper respiratory tract infections, nasopharyngitis, headache, injection site reactions, arthralgia, and diarrhea.
Clinical Research: Efficacy and safety were established in pivotal Phase 3 clinical trials (e.g., VOYAGE 1 and VOYAGE 2), which demonstrated significant improvements in psoriasis clearance compared to placebo.
6. Drug Interactions
Vaccines: Live vaccines should be avoided during treatment.
Other Immunosuppressants: The safety and efficacy of combining guselkumab with other immunosuppressants or phototherapy have not been evaluated.
CYP450 Substrates: The likelihood of clinically relevant drug interactions with drugs metabolized by CYP enzymes is low.

Composition: The active ingredient is guselkumab. Excipients include L-histidine, L-histidine hydrochloride monohydrate, polysorbate 80, sucrose, and water for injection.
Appearance: The solution is clear to opalescent and colorless to pale yellow.
Storage: Store in the original package at 2°C to 8°C (36°F to 46°F). Do not freeze or shake.
