1. Indications and Usage
Primary Indication: Indicated for lowering intraocular pressure (IOP) in adult patients with primary open-angle glaucoma or ocular hypertension who are inadequately controlled on the single agent.
Contraindications: Contraindicated in patients with known hypersensitivity to brinzolamide, brimonidine tartrate, sulfonamides, or benzalkonium chloride. It is also contraindicated in patients being treated with monoamine oxidase inhibitors (MAOIs) or drugs that affect noradrenergic transmission (e.g., tricyclic antidepressants).
2. Dosage and Administration
Recommended Dosage: The recommended dose is one drop in the affected eye(s) twice daily.
Missed Dose: If a dose is missed, it should be administered as soon as remembered. However, if it is almost time for the next dose, the missed dose should be skipped. Do not double the dose.
Multiple Topical Ophthalmic Drugs: When using multiple topical ophthalmic drugs, the administration of different eye drops should be separated by at least 10 minutes.
3. Mechanism of Action
Brinzolamide: A topical carbonic anhydrase inhibitor that reduces aqueous humor secretion by inhibiting carbonic anhydrase in the ciliary epithelium of the eye. It is a sulfonamide derivative.
Brimonidine Tartrate: A selective alpha-2 adrenergic agonist that lowers intraocular pressure by decreasing aqueous humor production and increasing uveoscleral outflow. The combination of these two agents provides a synergistic effect to reduce intraocular pressure.
4. Safety and Warnings
Ocular Allergic Reactions: Ocular allergic reactions, characterized by conjunctival hyperemia, ocular allergy, and keratitis, may occur.
Systemic Effects: Brimonidine can be systemically absorbed and cause central nervous system depression, including somnolence, fatigue, and dry mouth.
Sulfonamide Sensitivity: Brinzolamide is a sulfonamide, and systemic carbonic anhydrase inhibitors can cause adverse reactions such as skin rash, metabolic acidosis, and kidney stones.
Contact Lenses: The preservative, benzalkonium chloride, may be absorbed by soft contact lenses and cause ocular irritation. Patients should remove contact lenses before administration and wait at least 15 minutes before reinserting them.
5. Adverse Reactions and Clinical Research
Adverse Reactions: The most common adverse reactions include ocular hyperemia, ocular allergy, and dysgeusia (bitter or bad taste in the mouth after instillation). Other common reactions include ocular irritation, dry eye, and somnolence.
Clinical Research: Clinical trials have demonstrated that the combination of brinzolamide and brimonidine tartrate significantly lowers intraocular pressure compared to monotherapy, with a high proportion of patients achieving therapeutic targets.
6. Drug Interactions
MAOIs and Antidepressants: Concomitant use with MAOIs or tricyclic antidepressants is contraindicated due to the risk of severe systemic interactions.
Oral Carbonic Anhydrase Inhibitors: Concomitant use is not recommended due to the potential for an additive effect on systemic carbonic anhydrase inhibition.
CNS Depressants: Caution is advised when used with alcohol, barbiturates, or opioids, as brimonidine may enhance their effects.
Antihypertensives: Caution is advised when used with antihypertensives or drugs that affect heart rate (e.g., beta-blockers), as brimonidine may cause hypotension and bradycardia.
