Siweisi Clebopride Malate Tablets

1. Indications and Usage
Gastroesophageal Reflux: Treatment of chronic gastroesophageal reflux with heartburn and acid regurgitation, including patients who do not respond adequately to standard acid-suppressive therapy.
Chronic Constipation: Treatment of chronic constipation in patients who have not responded adequately to conventional therapy.
2. Dosage and Administration
Adults: The usual dosage is 1 tablet (1 mg) taken twice daily, in the morning and evening.
Administration: Tablets should be swallowed with water, preferably 15 minutes before meals.
Duration: The duration of treatment should be determined by the physician. If no improvement is seen after 2 weeks of treatment, the drug should be discontinued.
3. Mechanism of Action
Serotonin Receptor Agonism: Clebopride malate acts as a highly selective agonist at serotonin type 4 (5-HT4) receptors.
Acetylcholine Release: By stimulating these receptors on the presynaptic nerve terminals of the enteric nervous system, it promotes the release of acetylcholine.
Physiological Effects: This enhances gastrointestinal motility and decreases the lower esophageal sphincter pressure, thereby improving gastric emptying and facilitating bowel movements.
4. Safety and Warnings
Contraindications: Contraindicated in patients with gastrointestinal hemorrhage, mechanical obstruction, or perforation.
Cardiac Risk: There is a risk of cardiac arrhythmias, including QT interval prolongation and torsades de pointes. It should be used with caution in patients with prolonged QT interval, uncorrected hypokalemia, or bradycardia.
Special Populations: Safety and efficacy in children and adolescents under 18 years of age have not been fully established. Use with caution in pregnant and lactating women.
5. Adverse Reactions and Clinical Research
Common Adverse Reactions: Hyperhidrosis, headache, dizziness, abdominal pain, diarrhea, nausea, and vomiting.
Serious Risks: QT prolongation, ventricular arrhythmias, and hypokalemia.
Other Effects: Galactorrhea and gynecomastia may occur due to potential effects on prolactin levels.
6. Drug Interactions
QT-Prolonging Drugs: Concomitant use with drugs that prolong the QT interval (e.g., antiarrhythmics, antipsychotics) may increase the risk of cardiac arrhythmias.
CYP Enzyme Inhibitors: Drugs that inhibit CYP enzymes may alter the metabolism of clebopride.
Antacids: While not a major interaction, taking it 15 minutes before meals is recommended to optimize absorption and efficacy.
