1. Indications and Usage
Vitamin K Deficiency: Indicated for the prevention and treatment of bleeding due to vitamin K deficiency, such as in cases of malabsorption syndromes or prolonged antibiotic use.
Hypoprothrombinemia: Used to correct hypoprothrombinemia induced by vitamin K antagonists when specific reversal is required.
2. Dosage and Administration
Adults: The usual dosage is 1–2 mg (1–2 tablets) daily, taken orally. The dose may be increased to 4 mg daily in severe deficiency, but should be adjusted based on prothrombin time.
Children: Dosage is typically 0.5–1 mg daily, depending on age and severity of deficiency.
Administration: Can be taken with or without food. Treatment duration should be limited to the period of deficiency.
3. Mechanism of Action
Vitamin K Activity: Menadione is a synthetic analog of natural vitamin K. It participates in the carboxylation of glutamate residues on clotting factors II, VII, IX, and X.
Cofactor Function: This process enables these factors to bind calcium and function in the coagulation cascade, thereby promoting blood clotting.
4. Safety and Warnings
Hemolytic Anemia: High doses, particularly in newborns or infants with G6PD deficiency, can cause severe hemolytic anemia and hyperbilirubinemia (kernicterus).
Hepatic Toxicity: Large doses can cause liver damage, especially in patients with pre-existing liver disease.
Contraindications: Contraindicated in pregnant women, nursing mothers, and neonates/infants due to the risk of hemolysis and hyperbilirubinemia.
5. Adverse Reactions and Clinical Research
Common Adverse Reactions: Nausea, vomiting, abdominal pain, and hypersensitivity reactions (rash, pruritus).
Serious Adverse Reactions: Hemolytic anemia, thrombocytopenia, and acute hepatic necrosis (with overdose).
Clinical Findings: While effective in correcting hypoprothrombinemia, its use has largely been replaced by safer natural vitamin K analogs (e.g., phytonadione) in many clinical settings.
6. Drug Interactions
Anticoagulants: Antagonizes the effects of vitamin K antagonists (e.g., warfarin, acenocoumarol).
Salicylates: May increase the risk of bleeding when used concurrently with anticoagulants.
Cholestyramine: Can interfere with the absorption of vitamin K, reducing its efficacy.
