1. Indications and Usage
ENSPRYNG is indicated for the treatment of adult and pediatric patients 12 years of age and older with aquaporin-4 antibody-positive (AQP4-IgG positive) neuromyelitis optica spectrum disorder (NMOSD).
2. Dosage and Administration
Route: Subcutaneous injection (abdomen or thigh). Rotate injection sites.
Recommended Dose: 120 mg administered once every 4 weeks.
Initial Dosing Schedule: The first three doses should be administered at Week 0, Week 2, and Week 4. After the Week 4 dose, continue with 120 mg every 4 weeks.
Missed Dose: If a dose is missed, administer it as soon as possible within 4 weeks of the missed dose. Wait until 4 weeks after the last dose for the next scheduled dose.
Premedication: No premedication is required.
3. Mechanism of Action
Satralizumab is a human interleukin-6 (IL-6) receptor antagonist monoclonal antibody. It binds specifically to both soluble and membrane-bound IL-6 receptors, inhibiting IL-6 mediated signaling. This suppresses the inflammatory and immune-mediated damage associated with NMOSD.
4. Safety and Warnings
Infections: Increased risk of serious infections. Evaluate for active infections prior to each dose; delay treatment if an active infection is present. Screen for Hepatitis B and tuberculosis (TB) before initiation.
Hepatic Toxicity: Elevations in liver transaminases (ALT/AST) have been observed. Monitor liver function tests every 4 weeks for the first 3 months, then every 3 months for the first year.
Gastrointestinal Perforation: Occurred in patients with diverticulitis; consider in patients with new abdominal symptoms.
Vaccinations: Do not administer live or live-attenuated vaccines during treatment. Ensure all vaccinations are up to date before starting.
Embryo-Fetal Toxicity: May cause fetal harm.
5. Adverse Reactions and Clinical Research
Common Adverse Reactions: The most common adverse reactions (incidence ≥10%) include injection site reactions, nasopharyngitis, headache, upper respiratory tract infection, nausea, arthralgia, limb pain, fatigue, and rash.
Clinical Research: The efficacy and safety are based on a Phase 3 randomized, double-blind, placebo-controlled trial in AQP4-IgG positive NMOSD patients, demonstrating a significant reduction in the annualized rate of relapse.
6. Drug Interactions
Live Vaccines: Concomitant use with live vaccines is not recommended.
Immunosuppressives: The concomitant use with other immunosuppressive drugs has not been fully evaluated.
CYP450 Substrates: Satralizumab is not known to induce or inhibit CYP450 enzymes; clinically significant pharmacokinetic interactions with small-molecule drugs are unlikely.
7. Pharmaceutical Information
Active Ingredient: Satralizumab-mwge.
Strength: 120 mg/1 mL (single-dose prefilled syringe).
Description: Clear to slightly opalescent, colorless to pale yellow liquid.
Storage: Store in a refrigerator at 2°C to 8°C (36°F to 46°F) in the original carton to protect from light. Do not freeze or shake. If needed, it may be stored at room temperature up to 30°C (86°F) for up to 8 days.
