1. Indications and Usage
Unresectable or Metastatic Nasopharyngeal Carcinoma (NPC):
Indicated for the treatment of adult and pediatric patients (≥6 years) with recurrent or metastatic NPC who have progressed after platinum-based chemotherapy, or as first-line treatment in combination with cisplatin and gemcitabine.
Unresectable or Metastatic Melanoma:
Indicated for the treatment of adult patients with unresectable or metastatic melanoma who have progressed after prior therapy.
Locally Advanced or Metastatic Urothelial Carcinoma:
Indicated for the treatment of adult patients with locally advanced or metastatic urothelial carcinoma who have progressed during or following platinum-containing chemotherapy.
Recurrent or Metastatic Cervical Cancer:
Indicated in combination with chemotherapy for persistent, recurrent, or metastatic cervical cancer.
2. Dosage and Administration
Recommended Dosage:
Fixed Dose: 240 mg administered intravenously every 3 weeks.
Weight-based Dose (Specific Indications): 3 mg/kg administered intravenously every 2 weeks (e.g., for melanoma or urothelial carcinoma in certain regions).
Administration:
Administer as an intravenous infusion over 30 to 60 minutes. Do not administer as an intravenous push or bolus.
3. Mechanism of Action
PD-1 Blockade:
Toripalimab binds to the PD-1 receptor on T-cell surfaces and blocks its interaction with ligands PD-L1 and PD-L2.
Immunological Effect:
This blockade prevents the inhibition of T-cell proliferation and cytokine production, thereby enhancing anti-tumor immune responses and inhibiting tumor growth.
4. Safety and Warnings
Immune-Mediated Adverse Reactions:
Toripalimab can cause severe or fatal immune-mediated adverse reactions (e.g., pneumonitis, colitis, hepatitis, endocrinopathies). Monitor patients closely.
Infusion-Related Reactions:
Infusion-related reactions may occur. Monitor during infusion and interrupt or discontinue if severe.
Embryo-Fetal Toxicity:
May cause fetal harm. Advise females of reproductive potential to use effective contraception during treatment and for at least 2 months after the final dose.
5. Adverse Reactions
Most Common:
Fatigue, diarrhea, pyrexia, rash, pruritus, decreased appetite, cough, nausea, and increased transaminases.
6. Drug Interactions
Immunosuppressants:
Avoid systemic corticosteroids or other immunosuppressants prior to starting treatment as they may interfere with pharmacodynamic activity.
Metabolic Enzymes:
Toripalimab is not metabolized by CYP450 enzymes; therefore, drug interactions via metabolic pathways are not expected.
