1. Indications and Usage
GERD: As an alternative to oral therapy when oral therapy is not feasible.
Ulcer Bleeding: For the treatment of low-risk patients with acute gastric or duodenal ulcer bleeding (Forrest grades IIc-III) when oral therapy is not feasible.
2. Dosage and Administration
Recommended Dose: 20-40 mg once daily for GERD; 40 mg every 12 hours for 5 days for ulcer bleeding.
Duration: Usually for short-term use (not exceeding 7 days); switch to oral therapy as soon as possible.
Administration: Intravenous injection over at least 3 minutes, or IV infusion over 30 minutes.
3. Mechanism of Action
Target: Esomeprazole sodium is a specific inhibitor of gastric H+/K+-ATPase.
Action: It selectively blocks the final step of gastric acid production in parietal cells, reducing acid secretion.
4. Safety and Warnings
Hypomagnesemia: Severe hypomagnesemia has been reported with prolonged use (usually >3 months).
Bone Fracture: Long-term use may increase the risk of osteoporosis-related fractures.
CDAD: Use may alter intestinal flora and increase the risk of Clostridium difficile-associated diarrhea.
Renal and Lupus: Monitor for acute interstitial nephritis and new onset or worsening of lupus.

CYP2C19 Substrates: May increase plasma concentrations of drugs metabolized by CYP2C19 (e.g., diazepam, phenytoin).
Protease Inhibitors: Contraindicated with nelfinavir; not recommended with atazanavir or saquinavir.
