1. Indications and Usage
ALK-Positive NSCLC: Treatment of adult patients with locally advanced or metastatic NSCLC who have previously been treated with crizotinib and have progressed or are intolerant to crizotinib.
Diagnostic Requirement: Use of the drug requires confirmation of ALK positivity by a validated diagnostic test prior to initiation.
2. Dosage and Administration
Route of Administration: Oral administration ONLY.
Recommended Dosage: 225 mg once daily.
Critical Administration Instructions:
Timing: Administer at the same time each day, either in the fasting state or with food.
Swallowing: Capsules must be swallowed whole; do not crush, chew, or open.
Missed Dose: If a dose is missed and it has been more than 12 hours until the next scheduled dose, take the missed dose. Otherwise, skip the missed dose.
Vomiting: If vomiting occurs after taking a dose, do not take an extra dose; continue with the next scheduled dose.
Dose Modifications: For severe adverse reactions, reduce the dose to 200 mg once daily (first reduction) or 150 mg once daily (second reduction). Permanently discontinue if unable to tolerate 150 mg once daily.
3. Mechanism of Action
ALK Inhibition: Ensartinib is a potent, selective, and reversible inhibitor of the anaplastic lymphoma kinase (ALK) tyrosine kinase.
Pathway Blockade: It binds to the ALK kinase domain, inhibiting ALK-mediated downstream signaling pathways (e.g., PI3K/AKT and MAPK/ERK).
Antitumor Effect: This blockade inhibits tumor cell proliferation and promotes apoptosis. It exhibits activity against several crizotinib-resistant ALK mutant variants.
4. Safety and Warnings
Hepatotoxicity: Drug-induced hepatotoxicity (elevations in ALT, AST, and bilirubin) has been reported. Monitor liver function tests before initiation, regularly during treatment, and as clinically indicated.
Interstitial Lung Disease (ILD)/Pneumonitis: Monitor for new or progressive respiratory symptoms. Investigate promptly and manage according to severity (permanent discontinuation for Grade 2 or higher).
Cardiac Toxicity: QTc interval prolongation has been reported. Obtain baseline and periodic ECGs, especially in patients with cardiovascular risk factors.
Embryo-Fetal Toxicity: May cause fetal harm. Effective contraception is required for females and males with female partners during treatment and for at least 3 months after the final dose.
5. Adverse Reactions and Clinical Research
Most Common Adverse Reactions (≥10%): Skin and subcutaneous tissue disorders (rash, pruritus), gastrointestinal disorders (nausea, constipation, vomiting), and edema.
Common Laboratory Abnormalities: Increases in ALT, AST, serum creatinine, and γ-glutamyltransferase (γ-GT).
Clinical Research Highlights: The Phase 2 BTP-42322 trial demonstrated an overall response rate (ORR) of 74.4% in crizotinib-pretreated patients, with a manageable safety profile.
6. Drug Interactions
CYP3A4 Modulators: Ensartinib is primarily metabolized by CYP3A4. Strong CYP3A4 inhibitors may increase ensartinib exposure; strong CYP3A4 inducers may decrease exposure. Dose adjustments or avoidance is required.
Drugs Prolonging QTc Interval: Concomitant use with other drugs known to prolong the QTc interval may have an additive effect and should be avoided or managed with caution.
