1. Indications and Usage
Major Depressive Disorder: Treatment of major depressive disorder.
Generalized Anxiety Disorder: Treatment of generalized anxiety disorder.
Chronic Musculoskeletal Pain: Treatment of chronic pain associated with chronic musculoskeletal pain, including chronic low back pain and osteoarthritis pain.
2. Dosage and Administration
Depression: The recommended dose is 40 mg/day to 60 mg/day. Some patients may need to start at 30 mg/day for one week. Existing clinical data have not confirmed that doses exceeding 60 mg/day increase efficacy.
Generalized Anxiety Disorder: The recommended starting dose for adults is 60 mg/day. Elderly patients should start at 30 mg/day.
Chronic Musculoskeletal Pain: The recommended dose is 60 mg once daily, starting at 30 mg for one week.
Administration: The capsules should be swallowed whole. Do not chew, crush, or open the capsule shell. The drug can be taken with or without food.
Special Populations: For patients with severe renal impairment (estimated creatinine clearance <30 mL/min) or end-stage renal disease, or patients with hepatic impairment, the use of this drug is contraindicated.
3. Mechanism of Action
Neurotransmitter Reuptake Inhibition: Duloxetine is a selective serotonin and norepinephrine reuptake inhibitor. It blocks the reuptake of these neurotransmitters in the central nervous system.
Therapeutic Effects: By increasing the concentration of serotonin and norepinephrine in the synaptic cleft, it improves mood, relieves anxiety, and modulates pain signaling pathways.
4. Safety and Warnings
Suicidal Thoughts: Antidepressants increase the risk of suicidal thinking and behavior in children, adolescents, and young adults.
Serotonin Syndrome: Concomitant use with other serotonergic drugs may increase the risk of serotonin syndrome.
Liver Injury: Hepatotoxicity has been reported. Liver function tests should be monitored during treatment.
Blood Pressure and Heart Rate: Can cause increases in blood pressure and heart rate.
Withdrawal Syndrome: Abrupt discontinuation may cause adverse reactions, so gradual tapering is recommended.
Narrow-Angle Glaucoma: May cause mydriasis and should be avoided in patients with uncontrolled narrow-angle glaucoma.
