1. Indications and Usage
Serious Infections: Treatment of pneumonia (including hospital-acquired pneumonia), intra-abdominal infections, skin and soft tissue infections, meningitis, and septicemia caused by susceptible bacteria.
Gynecological Infections: Treatment of endometritis and pelvic inflammatory disease.
Complicated Urinary Tract Infections: Treatment of complicated urinary tract infections.
Empiric Therapy: Empiric treatment for febrile neutropenia in adult cancer patients.
2. Dosage and Administration
Adults: The usual dosage is 500 mg to 1 g administered every 8 hours. For meningitis, 2 g every 8 hours is recommended.
Pediatrics: Usually 10 to 20 mg/kg administered every 8 hours. For meningitis, 40 mg/kg every 8 hours (maximum 2 g per dose) may be used.
Renal Impairment: Dosage must be adjusted based on creatinine clearance to prevent drug accumulation.
Administration: Can be administered via intravenous push (over at least 5 minutes) or intravenous infusion (over 15 to 30 minutes).
3. Mechanism of Action
Cell Wall Inhibition: Meropenem is a bactericidal agent that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs).
Spectrum: It demonstrates broad-spectrum activity against aerobic and anaerobic Gram-positive and Gram-negative bacteria and is highly stable against many beta-lactamases.
4. Safety and Warnings
Hypersensitivity: Severe and occasionally fatal hypersensitivity reactions have been reported. It should not be used in patients with a history of immediate hypersensitivity to carbapenems.
Neurotoxicity: Seizures and other central nervous system adverse reactions have been reported, particularly in patients with CNS disorders or renal impairment.
Pseudomembranous Colitis: Broad-spectrum antibiotics may cause pseudomembranous colitis; use with caution in patients with a history of gastrointestinal disease.
Contraindication with Valproic Acid: Meropenem significantly decreases plasma concentrations of valproic acid and is contraindicated in patients taking valproic acid.
5. Adverse Reactions and Clinical Research
Common: Gastrointestinal symptoms such as diarrhea, nausea, vomiting, and injection site reactions.
Serious: Hypersensitivity reactions (including anaphylaxis), seizures, eosinophilia, transient elevation of liver enzymes, and pseudomembranous colitis.
Clinical Research: Pivotal clinical trials have demonstrated high efficacy and safety in treating severe community-acquired and nosocomial infections.
6. Drug Interactions
Valproic Acid: Concomitant use is contraindicated due to the risk of significant reduction in valproic acid levels and subsequent seizure recurrence.
Probenecid: Can delay the renal excretion of meropenem, increasing blood concentrations and half-life.
Aminoglycosides: Concomitant use may enhance the risk of nephrotoxicity.
