1. Indications and Usage
Secondary Prevention: Indicated for patients already on both clopidogrel and aspirin to prevent thrombotic events associated with atherosclerosis.
Acute Coronary Syndromes (ACS): Used for the transition of combination therapy in:
Non-ST-elevation acute coronary syndrome (unstable angina or non-Q wave myocardial infarction), including patients post percutaneous coronary intervention (PCI) with stent placement.
ST-elevation myocardial infarction patients suitable for thrombolytic therapy.
2. Dosage and Administration
Standard Dose: One tablet (75 mg clopidogrel and 100 mg aspirin) taken orally once daily.
Administration: The tablet should be swallowed whole with water and should not be crushed or chewed.
Duration:
Non-ST-elevation ACS: Clinical trials support a treatment duration of up to 12 months.
ST-elevation MI: Treatment should be initiated as early as possible and continued for at least 4 weeks.
Missed Dose: If taken within 12 hours of the usual time, take the missed dose immediately. If taken more than 12 hours late, skip the missed dose and resume the regular schedule. Do not double the dose.
3. Mechanism of Action
Clopidogrel: A prodrug metabolized by CYP450 enzymes to an active metabolite. It selectively and irreversibly inhibits the binding of ADP to the P2Y12 receptor on platelets, blocking ADP-mediated activation of the GPIIb/IIIa complex. This effect lasts for the platelet’s lifespan (approx. 7–10 days).
Aspirin: Irreversibly acetylates cyclooxygenase (COX), inhibiting the formation of thromboxane A2 (a promoter of platelet aggregation and vasoconstriction). This effect also lasts for the lifespan of the platelet.
4. Safety and Warnings
Bleeding Risk: The most significant risk is serious, sometimes fatal, bleeding. It is contraindicated in patients with active pathological bleeding (e.g., peptic ulceration, intracranial hemorrhage).
Gastrointestinal Toxicity: Aspirin can cause gastric irritation, ulcers, and bleeding.
Hepatic/Renal Impairment: Contraindicated in patients with severe hepatic dysfunction or severe renal impairment (CrCL <30 mL/min).
Pregnancy: Contraindicated in the third trimester due to risks of bleeding and premature closure of the ductus arteriosus.
Surgical Procedures: Due to increased bleeding risk, the drug may need to be discontinued prior to elective surgery if antiplatelet effect is not desired.
5. Adverse Reactions and Clinical Research
Common Adverse Reactions: Bleeding events (bruising, epistaxis), gastrointestinal pain, dyspepsia, and rash.
Serious Adverse Reactions: Major hemorrhage (gastrointestinal, intracranial), thrombotic thrombocytopenic purpura (TTP), severe neutropenia/agranulocytosis, and severe hypersensitivity reactions (anaphylaxis, angioedema).
Clinical Findings: Clinical trials (e.g., CURE study) have demonstrated that dual therapy significantly reduces the composite endpoint of cardiovascular death, myocardial infarction, or stroke compared to aspirin alone in ACS patients.
6. Drug Interactions
Warfarin and Other Anticoagulants: Significantly increases the risk of bleeding.
Non-Steroidal Anti-Inflammatory Drugs (NSAIDs): Concomitant use (e.g., ibuprofen, diclofenac) may increase the risk of gastrointestinal bleeding and may interfere with aspirin’s cardioprotective effects.
Proton Pump Inhibitors (PPIs): Omeprazole and esomeprazole may reduce the antiplatelet efficacy of clopidogrel by inhibiting CYP2C19 metabolism.
Serotonergic Agents: SSRIs/SNRIs may increase the risk of bleeding when combined with antiplatelet drugs.
