1. Indications and Usage
Arginine Glutamate Injection is indicated for the adjunctive treatment of hyperammonemia caused by chronic liver disease.
2. Dosage and Administration
Standard Dosage: The recommended daily dose for adults is 20 g (calculated as arginine glutamate).
Administration: It must be administered via slow intravenous infusion. The infusion time for 200 mL should be more than 2 hours.
Individualization: The dosage can be adjusted appropriately according to the patient’s age and symptoms.
3. Mechanism of Action
*k2>After intravenous administration, the drug dissociates into arginine and glutamic acid in the body. It has the effect of lowering blood ammonia and increasing liver arginase activity。
Arginine: Participates in the ornithine cycle to promote urea synthesis, converting ammonia into non-toxic urea for excretion in urine.
Glutamic Acid: Binds with excessive ammonia in the blood to form glutamine, which is then decomposed and excreted by the urine.
4. Safety and Warnings
Contraindications: Hypersensitivity to any component of the drug; patients with fulminant hepatic failure (due to lack of arginase in the body).
Renal Impairment: Use with caution in patients with severe renal dysfunction, as nitrogen-containing compounds may exacerbate existing symptoms.
General Precautions: Check the solution before use; do not use if it is not completely clear. Use immediately after opening; do not use any remaining solution.
5. Adverse Reactions and Clinical Research
Adverse Reactions: Although the incidence rate is unclear, reported reactions involve various systems. The most common include numbness, facial tightness, fever, dizziness, headache, nausea, vomiting, palpitations, chest discomfort, shortness of breath, flushing, and joint pain.
Clinical Research: Preclinical toxicity studies in rats did not show significant differences in weight changes, liver function, or liver pathology compared to the control group.
6. Drug Interactions
General Anesthetics (Barbiturates): May inhibit the metabolism and excretion of ammonia in the liver.
Isoniazid: May increase blood ammonia concentration.
Ion-Exchange Resins: May enhance ammonia toxicity.
Diuretics (Thiazides): Lower blood potassium levels, leading to compensatory increases in ammonia.
