1. Indications and Usage
Depression: Treatment of depression.
Generalized Anxiety Disorder: Treatment of generalized anxiety disorder.
2. Dosage and Administration
Starting Dose: The recommended starting dose for most patients is 75 mg once daily. For certain patients, starting at 37.5 mg daily for 4 to 7 days may be more suitable before increasing to 75 mg.
Maintenance Dose: The dose can be increased in increments of 75 mg/day at intervals of more than 4 days. The maximum dose is generally 225 mg/day.
Administration: Should be taken with food at the same time each day. The capsule should be swallowed whole and not chewed, crushed, or dissolved.
Special Populations: For patients with mild to moderate hepatic impairment, the total daily dose must be reduced by 50%. For patients with renal impairment, the dose must be reduced by 25% to 50% (50% for patients on dialysis).
3. Mechanism of Action
Reuptake Inhibition: Venlafaxine is a selective serotonin and norepinephrine reuptake inhibitor. It blocks the reuptake of these neurotransmitters in the central nervous system.
Therapeutic Effects: By increasing the concentration of serotonin and norepinephrine in the synaptic cleft, it improves mood and relieves anxiety.
4. Safety and Warnings
Suicidal Thoughts: Antidepressants increase the risk of suicidal thinking and behavior in children, adolescents, and young adults.
Serotonin Syndrome: Concomitant use with other serotonergic drugs may increase the risk of serotonin syndrome.
Blood Pressure and Heart Rate: Can cause increases in blood pressure and heart rate.
Withdrawal Syndrome: Abrupt discontinuation may cause adverse reactions, so gradual tapering is recommended.
Bleeding Risk: May increase the risk of bleeding, especially when used with aspirin or NSAIDs.
5. Adverse Reactions and Clinical Research
Common Adverse Reactions: Nausea, dry mouth, constipation, decreased appetite, fatigue, somnolence, and increased sweating.
Clinical Research: Clinical trials have demonstrated the efficacy of venlafaxine in treating depression and generalized anxiety disorder.
6. Drug Interactions
MAOIs: Concomitant use with monoamine oxidase inhibitors is contraindicated.
CYP2D6 Inhibitors: Strong CYP2D6 inhibitors may increase venlafaxine plasma concentrations.
CNS Depressants: Concomitant use with alcohol or other CNS depressants may potentiate sedation.
NSAIDs: Concomitant use with aspirin or NSAIDs may increase the risk of bleeding.
