1. Indications and Usage
Bronchospasm: Relieve bronchospasm associated with bronchial asthma, chronic bronchitis, emphysema, and other pulmonary diseases.
2. Dosage and Administration
Adults and Children ≥20 kg: The recommended dose is 5 mg via nebulizer, administered up to three times daily.
Children <20 kg: The recommended dose is 2.5 mg via nebulizer, administered up to four times daily.
Administration: Doses should be individualized and administered only via a nebulizer. The solution does not need to be diluted prior to use. Nebulizer technique should be checked regularly to ensure proper delivery.
3. Mechanism of Action
β2-Receptor Agonism: Terbutaline is a selective adrenergic β2-receptor agonist that works by selectively stimulating β2-receptors to relax bronchial smooth muscle and dilate airways.
Mucociliary Clearance: It can increase mucociliary clearance function reduced by obstructive lung disease, thereby accelerating the clearance of mucus secretions.
4. Safety and Warnings
Cardiovascular Disorders: Use with caution in patients with severe cardiovascular disorders (e.g., ischemic heart disease, rapid arrhythmias, or severe heart failure) due to the risk of cardiovascular symptoms.
Hypokalemia: β2-agonists may cause hypokalemia. Concomitant use with xanthine derivatives, steroids, and diuretics may exacerbate this effect.
Pregnancy and Lactation: Use with caution during the first trimester of pregnancy and at term due to uterine smooth muscle relaxation effects. The drug is excreted in breast milk.
Paradoxical Bronchospasm: If paradoxical bronchospasm occurs, the drug must be discontinued immediately.
5. Adverse Reactions and Clinical Research
Common Adverse Reactions: Tremor, headache, tachycardia, palpitations, muscle cramps, and nervousness.
Rare Adverse Reactions: Hypokalemia, lactic acidosis, and hypersensitivity reactions.
Clinical Research: Inhaled terbutaline acts within minutes and the effect lasts for approximately 6 hours. Systemic adverse reactions are rare when used at recommended doses.
6. Drug Interactions
Beta-Blockers: Non-selective beta-blockers (including ophthalmic preparations) may partially or completely inhibit the effect of terbutaline.
Potassium Depleting Agents: Concomitant use with diuretics, methylxanthines, and corticosteroids increases the risk of hypokalemia and arrhythmias.
Halogenated Anesthetics: Concomitant use may increase the risk of arrhythmias; terbutaline should ideally be discontinued at least 6 hours before halogenated anesthesia.
