1. Indications and Usage
Hypertension and Angina: Indicated for the treatment of hypertension and angina pectoris.
Chronic Heart Failure: Indicated for the treatment of chronic stable heart failure (NYHA class II-IV) with reduced left ventricular systolic function.
2. Dosage and Administration
Standard Dose: Usually taken once daily in the morning, with or without food. The tablet should be swallowed whole.
Hypertension/Angina: Initial dose is typically 5 mg once daily, which may be increased to 10 mg if necessary. Mild hypertension may be initiated at 2.5 mg.
Heart Failure: Must be started at a low dose (1.25 mg) and titrated up gradually every 4 weeks (to 2.5 mg, 3.75 mg, 5 mg, 7.5 mg) to a target of 10 mg, provided the patient tolerates the dose.
Renal/Hepatic Impairment: No dose adjustment is usually required for mild-to-moderate impairment, but caution is advised in severe cases.
3. Mechanism of Action
Beta-1 Blockade: Bisoprolol is a highly selective beta-1 adrenergic receptor blocker. It has no significant intrinsic sympathomimetic or membrane-stabilizing activity.
Cardiovascular Effects: By blocking cardiac beta-1 receptors, it reduces heart rate, myocardial contractility, and cardiac output, thereby decreasing myocardial oxygen demand.
Long-term Effects: Long-term use leads to a reduction in peripheral vascular resistance_Congestive Heart Failure_which contributes to its antihypertensive effect.
4. Safety and Warnings
Contraindications: Contraindicated in patients with cardiogenic shock, decompensated heart failure requiring IV inotropic therapy, second- or third-degree AV block, sick sinus syndrome, severe bradycardia, symptomatic hypotension, bronchial asthma, severe chronic obstructive pulmonary disease (COPD), and pheochromocytoma.
Abrupt Discontinuation: Do not stop abruptly in patients with ischemic heart disease, as it may precipitate angina or myocardial infarction.
Masking Hypoglycemia: May mask the tachycardia associated with hypoglycemia in diabetic patients.
5. Adverse Reactions and Clinical Research
Common Adverse Reactions: Bradycardia, hypotension, fatigue, cold extremities, heart failure exacerbation, and gastrointestinal disturbances.
Respiratory and Metabolic: Bronchospasm (in susceptible patients), dyspnea, hyperuricemia, and hypertriglyceridemia.
Neurological and Psychiatric: Dizziness, headache, sleep disturbances, and depression.
6. Drug Interactions
Other Beta-Blockers: Should not be used concurrently with other beta-blockers.
Calcium Channel Blockers: Concomitant use with verapamil or diltiazem is generally not recommended due to additive effects on heart rate and contractility.
Antiarrhythmics: Use with Class I antiarrhythmics (e.g., quinidine, disopyramide) should be done with caution.
Clonidine: If used concurrently, bisoprolol should be withdrawn before clonidine to avoid rebound hypertension.
