1. Indications and Usage
Hypothyroidism: Treatment of primary or secondary hypothyroidism (including congenital and acquired).
TSH Suppression: Suppression and treatment of benign thyroid nodules and thyroid cancer (post-surgical TSH suppression therapy).
Myxedema: Treatment of myxedema coma.
2. Dosage and Administration
Adults: Usually 100 to 200 mcg once daily. Dosage is adjusted based on TSH levels.
Elderly: Start with a low dose (25 to 50 mcg) and titrate up gradually.
Children: Higher doses per body weight are required (10 to 15 mcg/kg/day for infants).
Administration: Take orally on an empty stomach, 30 to 60 minutes before breakfast.
3. Mechanism of Action
Hormone Replacement: Levothyroxine is a synthetic form of thyroxine (T4). It is converted to triiodothyronine (T3) in tissues, regulating metabolism, growth, and development.
4. Safety and Warnings
Cardiovascular Risk: Untreated or overtreated hypothyroidism increases cardiovascular risk. Start low in elderly or those with cardiac disease.
Adrenal Insufficiency: Must rule out/treat adrenal insufficiency before starting to prevent adrenal crisis.
Pregnancy: Dosage requirements often increase significantly during pregnancy.
5. Adverse Reactions and Clinical Research
Hyperthyroid Symptoms: Occur primarily with overdose (palpitations, tachycardia, weight loss, heat intolerance, insomnia).
Other Effects: Headache, tremor, and diarrhea may occur.
6. Drug Interactions
Absorption Inhibitors: Absorption reduced by iron salts, calcium carbonate, antacids, and PPIs.
Metabolism Inducers: Phenobarbital, rifampin, and carbamazepine increase levothyroxine clearance.
Anticoagulants: May potentiate the effect of oral anticoagulants (e.g., warfarin).

Active Ingredient: Levothyroxine Sodium (C15H10I4NaO4).
Appearance: White tablets, color varies by strength (e.g., pink for 50 mcg, brown for 100 mcg).
