1. Indications and Usage
Community-Acquired Pneumonia: Indicated for the treatment of mild to moderate community-acquired pneumonia (CAP) caused by sensitive bacteria in adult patients (≥18 years old).
2. Dosage and Administration
Route of Administration: Intravenous infusion.
Recommended Dosage: 0.5 g once daily.
Infusion Details: The infusion must be administered slowly, with each infusion lasting no less than 90 minutes.
Treatment Duration: Treatment is typically continued for 7 to 14 days, depending on the severity of symptoms and clinical response.
Renal/Hepatic Impairment: No dose adjustment is required for patients with creatinine clearance ≥50 mL/min. No adjustment is needed for mild to moderate hepatic impairment.
3. Mechanism of Action
DNA Gyrase Inhibition: Nemonoxacin is a non-fluorinated quinolone that inhibits bacterial DNA gyrase (topoisomerase II).
Topoisomerase IV Inhibition: It also inhibits topoisomerase IV.
Bactericidal Effect: The inhibition of these two essential enzymes prevents DNA replication and transcription, exerting a bactericidal effect against pathogens.
4. Safety and Warnings
QT Prolongation: Quinolones may prolong the QT interval; caution is advised in patients with a history of QT prolongation or those taking antiarrhythmics.
Tendon Disorders: Quinolones are associated with tendonitis and tendon rupture; discontinue immediately at the first sign of tendon pain or inflammation.
CNS Effects: May cause dizziness, confusion, or seizures. Use with caution in patients with a history of epilepsy.
Photosensitivity: Photosensitivity reactions have been reported; patients should avoid excessive exposure to sunlight or UV light.
5. Adverse Reactions and Clinical Research
Most Common Adverse Reactions: Cardiac disorders (bradycardia, palpitations), skin reactions (rash, pruritus), gastrointestinal disturbances, and muscle spasms.
Clinical Research Highlights: Clinical trials have demonstrated that nemonoxacin is effective in clearing pathogens and resolving clinical symptoms in patients with community-acquired pneumonia.
6. Drug Interactions
Chelating Agents: Antacids, iron, calcium, magnesium, or zinc supplements may reduce absorption if taken orally concurrently; they should be administered at least 2 hours after nemonoxacin.
NSAIDs: Concomitant use with non-steroidal anti-inflammatory drugs (NSAIDs) may increase the risk of CNS excitation and seizures.
Theophylline: Potential interaction leading to increased serum theophylline levels and associated adverse effects.
