1. Indications and Usage
Type 1 Gaucher Disease: Indicated for the treatment of adult patients with mild to moderate type 1 Gaucher disease who are not candidates for enzyme replacement therapy.
Niemann-Pick Disease Type C: Indicated for the treatment of progressive neurological manifestations in adult and pediatric patients with Niemann-Pick disease type C.
2. Dosage and Administration
Recommended Dosage: 100 mg taken orally three times daily with food.
Pediatric Dosage: For patients under 18 years, the dose is adjusted according to body surface area.
Renal Impairment: In mild renal impairment (CrCl 50-70 mL/min), reduce to 100 mg twice daily. In moderate renal impairment (CrCl 30-50 mL/min), reduce to 100 mg once daily. It is not recommended for severe renal impairment (CrCl <30 mL/min).
Administration Details: Capsules should be swallowed whole. If diarrhea occurs, the dosage must be temporarily reduced.
3. Mechanism of Action
Enzyme Inhibition: Miglustat is a reversible competitive inhibitor of glucosylceramide synthase.
Substrate Reduction: By inhibiting this enzyme, it reduces the biosynthesis of glycosphingolipids, thereby lowering the levels of glycosphingolipid substrates that accumulate in lysosomes.
4. Safety and Warnings
Peripheral Neuropathy: Peripheral neuropathy (pain, weakness, numbness, paresthesia) has been reported. Neurological assessments should be performed at baseline and monitored regularly.
Tremor: Tremor or worsening of existing tremor is very common and may require dose reduction.
Gastrointestinal Disorders: Severe diarrhea, abdominal pain, and weight loss are common adverse reactions.
Thrombocytopenia: Mild reduction in platelet count may occur without bleeding complications.
Renal Impairment: Systemic exposure increases in patients with renal impairment; dose adjustments are mandatory based on creatinine clearance.
5. Adverse Reactions and Clinical Research
Most Common Adverse Reactions: Diarrhea, weight loss, tremor, abdominal pain, flatulence, nausea, headache, and dizziness.
Clinical Research: Clinical trials have demonstrated that miglustat reduces organomegaly and hematological abnormalities in type 1 Gaucher disease and slows the progression of neurological symptoms in Niemann-Pick disease type C.
6. Drug Interactions
Imiglucerase: Miglustat may increase the clearance of imiglucerase by 70%.
General Profile: No significant interactions with cytochrome P450 enzymes have been identified, but caution is advised with other drugs affecting the gastrointestinal tract or nervous system.
